摘 要:目的比较经皮椎体成形术(PVP)与椎体后凸成型术(PKP)治疗老年骨质疏松性椎体压缩性骨折(OVCFs)的疗效。方法将72例(83个椎体)老年骨质疏松性压缩性椎体骨折患者随机分为2组,分别行PVP术与PKP术。结果 PVP组渗漏率为36. 10%,PKP组渗漏率为13. 90%,2组差异有统计学意义(P <0. 05)。2组每个患椎注入骨水泥量、平均椎体高度恢复率比较有显著差异(P <0. 01)。结论 PVP与PKP均能有效治疗骨质疏松性椎体压缩性骨折,其中PVP操作相对容易,费用稍低,但骨水泥渗漏发生率高。PKP在椎体骨折复位及后凸畸形矫正方面具有优势。Objective To compare the clinical efficiency of percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of elderly patients with osteoporotic vertebral compression fractures (OVCFs). Methods Totally 72 patients (83 vertebrae) with OVCFs were randomly divided into PVP group and PKP group. Results The leakage rate of PVP group was 36.10%, which was significantly higher than 13.90% of PKP group ( P 〈0.05). There were significant differences in bone cement volume and average vertebral height recovery rate between the two groups ( P 〈0.01). Conclusion PVP and PKP are effective in the treatment of osteoporotic vertebral compression fractures. PVP is relatively easy to operate with a slightly lower cost, but the incidence rate of bone cement leakage is higher. PKP has advantages in vertebral fracture reduction and kyphotic deformity correction.
