黄体功能不足性月经失调患者行蔡氏妇科滋肾调冲汤联合针灸治疗的临床价值

摘　　要：目的:探讨滋肾调冲汤联合针灸治疗黄体功能不足性月经失调的临床效果。方法:选取2016年6月-2018年6月本院门诊收治的黄体功能不足性月经失调患者80例作为研究对象,按照随机数字表法将其分为对照组和中医组,各40例。对照组单用滋肾调冲汤,中医组联合应用滋肾调冲汤与中医针灸疗法,比较两组临床疗效、中医证候评分、黄体中期孕酮水平、高低温相温差(HPS)评分及不良反应发生情况。结果:治疗后两组月经量少、月经质稠色红、口干便结、头晕耳鸣、腰膝酸软评分及总分与治疗前比较均明显降低,且中医组均明显低于对照组,差异均有统计学意义(P<0.05)。治疗后两组黄体中期孕酮水平及HPS评分与治疗前比较均明显升高,且中医组均明显高于对照组,差异均有统计学意义(P<0.05)。中医组治疗总有效率为95.0%明显高于对照组的75.0%,差异有统计学意义(x^2=15.686,P<0.05)。两组不良反应发生情况比较,差异无统计学意义(P>0.05)。结论:滋肾调冲汤联合中医针灸治疗黄体功能不足引起的月经失调能够有效改善患者的临床症状,提高孕酮水平,治疗效果显著且无明显不良反应。Objective：To explore the clinical effect of Zishen-Tiaochong Decoction combined withacupuncture and moxibustion in the treatment of luteal insufficiency menstrual disorder.Method：A total of 80 patients with luteal insufficiency menstrual disorder admitted in the outpatient clinic of our hospital from June 2016 to June 2018 were selected as the study objects.According to the random number table method,they were divided into control group and TCM group,40 cases in each group.The control group was treated with Zishen-Tiaochong Decoction alone,the TCM group was treated with Zishen-Tiaochong Decoction combined with TCMacupuncture and moxibustion.The clinical efficacy,TCM syndrome score,progesterone level in mid-luteal phase,HPS score and adverse reactions were compared between the two groups.Result：After treatment, the menstrual volume less,thick red menstruation,dry mouth and stool,dizziness tinnitus,lumbar and knee acerbity softscores and total scores of the two groups were significantly lower than those before reatment,and the TCM group were significantly lower than those of the control group,the differences were statistically tically significant（ P〈0.05）.After treatment, the progesterone levels and HPS scores in the mid-luteal phase of the two groups were significantlyhigher than those before treatment,and the TCM group were significantly higher than those in the control group,the differences were statistically significant（ P〈0.05）.The total effective rate of the TCM group was 95.0%,whichwas significantly higher than 75.0% of the control group,the difference was statistically significant（ x2=15.686,P〈0.05）.There was no significant difference in the incidence of adverse reactions between the two groups（ P〉0.05）.Conclusion：Zishen-Tiaochong Decoction combined with acupuncture and moxibustion of traditional Chinesemedicine can effectively improve the clinical symptoms and progesterone levels of patients with menstrual disorderscaused by deficiency of luteal function.The therapeutic effe
