玻璃化冷冻胚胎-解冻移植周期中囊胚和卵裂期胚胎的妊娠结果分析

摘　　要：目的通过回顾性研究,分析囊胚和卵裂期胚胎玻璃化冷冻解冻移植周期的妊娠结果影响。方法以2016年1月-2017年10月新疆医科大学第一附属医院生殖助孕中心作为研究阶段开展回顾性分析,选择需玻璃化冷冻解冻患者,根据选择囊胚和卵裂期胚胎分为卵裂组、囊胚组,进行囊胚培养、玻璃化冷冻、胚胎的玻璃化解冻、囊胚培养,对比妊娠结局。结果两组年龄、不孕时间、移植周期数、HCG内膜厚度、平均移植胚胎数差异不显著,无统计学意义,P>0.05。两组患者的复苏率、多胎率、流产率、异位妊娠率比较无显著差异(P>0.05);囊胚组的妊娠率55.24%(58/105)、种植率50.79%(96/189)均要优于卵裂组的妊娠率31.25%(120/384)、种植率23.36%(75/321),P<0.05,具有统计学意义。结论玻璃化冷冻解冻是一种效果明显的冷冻方法,对于囊胚和卵裂期胚胎有良好的复苏效果,但是囊胚冷冻移植的临床效果更加明显。Objective The retrospective study was designed to evaluate the effects of pregnancy outcomes after vitrification at the cleavage stage and blastocyst stage for embryo transfer.Methods A retrospective analysis was conducted with patients in Reproductive Medical Center of the first affiliated hospital of Xinjiang Medical University from January 2016 to 2017 October.The patients who need vitrification and thawing were selected and divided into cleavage group and blastocyst group followed by cycle of Blastocyst culture,vitrification freeze,embryo glass thawing,blastocyst culture.The pregnancy outcomes were compared between those two groups.Results There were no significant differences in age,infertility time,transplant cycle number,HCG intima-Film thickness,average transplanted embryo number in two groups（P〉0.05）.There was no significant difference in resuscitation rate,multiple fetal rate,miscarriage rate,ectopic pregnancy rate neither（P〉0.05）.However,pregnancy rate[55.24%（58/105）]and planting rate[50.79%（96/189）]in the Blastocyst group were significantly better than that in the cleavage group[31.25%（120/384）]and[23.36%（75/321）]respectively（P〈0.05）.ConclusionVitrification is a kind of freezing method,which has a good effect on blastocyst and cleavage stage embryo,but the clinical outcome of blastocyst stage is more significant.
