摘 要:目的:观察灯盏益肾颗粒对糖尿病肾病(Diabetic nephropathy,DN)大鼠肾脏组织病理的影响,探讨灯盏益肾颗粒对DN可能的作用靶点和机制,为实验用药治疗DN提供依据。方法:雄性SD大鼠90只随机分成正常组(A组)、模型组(B组)、盐酸贝那普利组(C组)、灯盏益肾低、中、高剂量组(D、E、F组)。链脲佐菌素(STZ) 55 mg/kg一次性腹腔注射建立DN模型,给予不同实验因素干预后,观察实验大鼠的一般情况;测定尿微量白蛋白/尿肌酐(Urine Microalbumin to Urine Creatinine,ACR)、胱抑素C (CystatinC,CysC)、肌酐(Serum Creatinine,SCr)、尿素氮(Blood Uria Nitrogen,BUN)等指标;取肾脏组织,HE、PAS、Masson染色后,用光镜观察各组大鼠的肾脏组织病理改变。结果:与本组造模前比较,B、C、D、E、F组大鼠造模后的血糖和24-hrUP升高(P <0.05)。与A组比较,B、C、D、E、F组大鼠血糖和24 h尿蛋白(24-hour Urinary Protein,24-hrUP)升高(P <0.05);B组ACR明显升高(P <0.01),肾小球系膜基质增生、肾小球基底膜(GBM)增厚、肾小管管腔扩张、肾小管上皮细胞空泡样变性明显;肾皮质坏死、肾小管坏死、肾组织炎性细胞浸润明显(P <0.01)。与B组比较,C、D、E、F组ACR、BUN、CysC降低,肾小球系膜基质增生、GBM增厚、肾小管管腔扩张、肾小管上皮细胞空泡样变性改善(P <0.05,P <0.01);D、E、F组肾皮质坏死、肾小管坏死、肾组织炎性细胞浸润改善(P <0.05);与C组比较,D、E、F组肾皮质坏死、肾小管坏死、肾组织炎性细胞浸润改善(P <0.05);与C、D组比较,E、F组ACR、CysC降低(P <0.05);E、F组肾小球系膜基质增生、GBM增厚、肾小管管腔扩张、肾小管上皮细胞空泡样变性改善(P <0.05)。结论:灯盏益肾颗粒可减少糖尿病肾病大鼠ACR排泄量、减轻肾功能损伤、减轻肾脏组织病理病变。上述效应在一定范围内呈剂量依赖性。Objective: To observe the effect of Dengzhan Yishen granules on the pathology of the kidney tissues of rats with diabetic nephropathy(DN), and discuss the possible target and mechanism of Dengzhan Yishen granules on DN, so as to provide the basis for experimental medicine in treating DN. Methods- Divided 90 cases of male SD rats into the normal group (Group A), the model group(Group B), the benazepril hydrochloride group(Group C), and the Dengzhan Yishen groups of low, middle and high dose(Group D, E and F) randomly. The DN models were established by one-time intraperitoneal injection of 55 mg/kg of streptozotocin(STZ), and the general situation of the experimental rats was observed after the intervention of different experimental factors. Some indicators such as Urine Microalbumin to Urine Creatinine(ACR), CystatinC(CysC), Serum Creatinine (SCr), Blood Uria Nitrogen (BUN) were measured. After staining of hematoxylin-eosin (HE), periodic acid-schiff(PAS) and Masson, the pathological changes of the kidney tissues of rats in every group were observed by light microscopy. Results.- Levels of blood glucose and 24-hrUP were increased in Group B, C, D, E and F after model establishment when compared with those before establishment in each group respectively(P 〈 0.05). Compared with those in Group A, levels of blood glucose and 24-hour urinary protein(24-hrUP) in Group B, C, D, E and F rose(P 〈 0.05). In Group B, the level of ACR was significantly increased(P 〈 0.01); hyperplasia of mesangial stromal matrix of glomerulus, thickening of glomerular basement membrane(GBM), dilatation of renal tubular lumen, and vacuolar degeneration of renal tubular epithelial cells were obvious; renal cortical necrosis, renal tubular necrosis and renal inflammatory cell infiltration were obvious(P 〈 0.01).
