鼻内镜下OSAHS温控射频消融术对儿童OSAHS治疗效果观察

摘　　要：目的回顾性分析鼻内镜下阻塞性睡眠呼吸暂停低通气综合征(OSAHS)温控射频消融术(腺样体+扁桃体切除术)对儿童OSAHS的疗效。方法 120例OSAHS患儿,按照硬币法分为等离子组(80例)与传统组(40例)。等离子组采取温控射频消融术(腺样体+扁桃体切除术)治疗,传统组采取传统鼻内镜下动力系统切除腺样体+扁桃体剥离术联合腺样体刮除术治疗。对比两组患儿的治疗效果及术中出血量、手术时间、白膜脱落时间、症状缓解时间、术后疼痛时间。结果两组患儿治疗总有效率均为100.0%,比较差异无统计学意义(P>0.05)。等离子组患儿的手术时间(35.1±6.1)min、白膜脱落时间(8.7±2.6)d、症状缓解时间(2.0±2.4)d、术后疼痛时间(1.6±0.5)d均显著短于传统组的(50.0±7.7)min、(9.9±2.0)d、(3.7±3.0)d、(3.4±1.7)d,术中出血量(1.0±0.5)ml少于传统组的(5.9±2.0)ml,差异具有统计学意义(P<0.05)。结论对OSAHS患儿行鼻内镜下OSAHS温控射频消融术(腺样体+扁桃体切除术)治疗可取得理想的疗效,且手术操作简便、术后恢复快、安全性高,值得临床推广应用。Objective To analyze retrospectively the efficacy of obstructive sleep apnea hypopnea syndrome（OSAHS） temperature controlled radiofrequency ablation under nasal endoscopy（adenoidectomy ＋ tonsillectomy） on children with OSAHS. Methods A total of 120 children with OSAHS were divided into plasma group for 80 cases and traditional group for 40 cases according to the method of coins. The plasma group was treated with temperature controlled radiofrequency ablation（adenoidectomy ＋ tonsillectomy）. The traditional group was treated with dynamic system adenoidectomy and tonsillectomy by dissection combined with curettage of adenoids under traditional nasal endoscopy. The therapeutic effect, intraoperative blood loss, operative time, white membrane abscission time, symptom relief time and postoperative pain time between the two groups were compared. Results The total effective rate of the two groups were 100.0%, and there was no significant difference between the two groups（P〉0.05）. The operative time, white membrane abscission time, symptom relief time and postoperative pain time of the plasma group were（35.1±6.1） min,（8.7±2.6） d,（2.0±2.4） d and（1.6± 0.5） d respectively, which were（50.0±7.7） min,（9.9±2.0） d,（3.7±3.0） d and（3.4±1.7） d of the traditional group, the intraoperative blood loss of the plasma group was（1.0±0.5） ml, which was significantly less than（5.9±2.0） ml of the traditional group, the differences were statistically significant（P〈0.05）. Conclusion The temperature controlled radiofrequency ablation（adenoidectomy ＋ tonsillectomy） of OSAHS under nasal endoscopy can achieve ideal curative effect in children with OSAHS, and the operation is simple, with quick postoperative recovery and high safety, it is worthy of clinical promotion and application.
