摘　　要：目的:了解郑州人民医院(以下简称"我院")Ⅰ类切口手术围术期预防性应用抗菌药物的情况,为临床合理用药提供参考。方法:调取2017年我院所有Ⅰ类切口手术病历,共2 738份,从抗菌药物的品种选择、给药时间、用法与用量及疗程等方面进行回顾性统计分析,并评估用药合理性。结果:2 738例Ⅰ类切口手术中,围术期预防性应用抗菌药物739例,预防性用药率为26.99%。739例预防性应用抗菌药物病例中,合理用药391例(占52.91%),不合理用药348例(占47.09%);不合理用药类型主要包括疗程过长(229例,占不合理用药病例数的65.80%)、药品选择不适宜(91例,占不合理用药病例数的26.15%)。结论:我院Ⅰ类切口手术围术期抗菌药物预防性使用率基本合理,但仍存在疗程过长、药品选择不适宜等问题,应定期进行抗菌药物合理应用培训,促进临床用药。OBJECTIVE： To investigate the perioperative prophylactic antibiotics in type Ⅰ incision surgery in People＇s Hospital of Zhengzhou（hereinafter referred to as ＂our hospital＂）, so as to provide reference for the rational drug application in clinic. METHODS： 2 738 medical records of type Ⅰ incision surgery in 2017 were extracted from our hospital, retrospective statistical analysis was conducted on selection, time of administration, usage and dosage, and course of treatment of antibiotics, etc. And the drug application rationality was evaluated. RESULTS： In 2 738 cases of type Ⅰ incision surgery, 739 cases of prophylactic antibiotics were used during the perioperative period, and the prophylactic drug use rate was 26.99%. Among 739 cases of prophylactic antibiotics, 391 cases were used rationally（52.91%）, and 348 cases were irrational（47.09%）. The types of irrational drug application mainly performed as long course of treatment（229 cases, 65.80%）, inappropriate drug selection（91 cases, 26.15%）. CONCLUSIONS： The application rate of perioperative prophylactic antibiotics in type Ⅰ incision surgery in our hospital is basically rational, but there are still some problems such as long course of treatment and inappropriate drug selection. Regular training in the application of antibiotics should be carried out to promote the clinical drug application.
