摘　　要：文章综述了国内外老年心房颤动患者的抗凝治疗、抗凝的影响因素、抗凝医养管理的现状及效果,提出医养结合模式下多学科一体化管理模式、健康教育、远程信息技术在抗凝管理中的应用等,旨在为我国医养结合模式下如何利用有限的资源做好老年房颤患者的抗凝管理提供新的思路.This paper reviewed anticoagulant therapy of elderly patients with atrial fbrillation （AF） at home and abroad, the infuence factors of anticoagulation, the present situation and effect of medical treatment and care management, put forward the application of multidisciplinary integration system management mode, health education, remote information technology under integrated service of medical treatment and care in anticoagulant management, provided new ideas for how to do the anticoagulation management of elderly patients with atrial fbrillation with the limited resources under integrated service of medical treatment and care in our country.
