循经按摩缓解脑卒中后痉挛性偏瘫临床观察

摘　　要：目的:运用循经按摩缓解脑卒中后痉挛性偏瘫,提高患者康复效果,降低致残率,提高生活质量。方法:70例随机分为常规康复组和循经按摩组,常规康复组采用良肢位摆放、主被动活动、肢体空气压力治疗、站立训练等康复治疗,循经按摩组在常规康复治疗的基础上增加循经按摩,分别于治疗2周、4周后运用改良Ashworth量表评价2组患者肘关节、膝关节的肌张力状况。结果:循经按摩组患者的上肢肘关节、下肢膝关节的肌张力改善明显优于常规康复组。比较2组临床效果,治疗2周后循经按摩组总有效率82.85%,常规康复组为60%,循经按摩组优于常规康复组(P<0.05)。治疗4周后循经按摩组总有效率88.57%,常规康复组为65.71%,循经按摩组优于常规康复组(P<0.05)。结论:循经按摩可有效地缓解偏瘫痉挛患者肢体痉挛程度,降低肌张力,促进患者肢体功能恢复,使患者的运动功能和生活自理能力得到更快的恢复。Objective： To relieve spasmodic hemiplegia after stroke by massage and reduce the disability rate and improve the quality of life. Methods： 70 patients with spastic hemiplegia after stroke according to random number table method was divided into routine rehabilitation group and massage. The routine rehabilitation group used good limb po- sition to put, main passive activity, body air pressure treatment and standing training rehabilitation. The massage group on the basis of routine rehabilitation treatment increased tired massage. Using the modified Ashworth scale evaluated the muscular tension of elbow and knee in both groups in 2 weeks and 4 weeks after treatment. Results： The muscle tension of the upper elbow and lower limb knee of the massage group was better than that of the routine rehabilitation group. Comparing the clinical effects of the two groups, the total effective rate of the massage group was 82.85% and the routine rehabilitation group was 60% after 2 weeks of treatment, so the massage group was better（P〈0.05）. After 4 weeks of treatment, the total effective rate of the massage group was 88.57%, that of the routine rehabilitation group was 65.71%, and that of the massage group was better than that of the conventional rehabilitation group（P〈0.05）. Conclu- sion： Massage can effectively alleviate the degree of hemiplegia spasm in patients with limb spasm, reduce muscle ten- sion, promote the limb functional recovery, make the movement function of the patients with self-care ability and life get faster recovery.
