摘 要:目的探讨利拉鲁肽对2型糖尿病合并冠心病患者血清炎症因子及同型半胱氨酸水平的影响。方法选取石家庄市第二医院2014年10月-2016年10月接诊的2型糖尿病合并冠心病患者126例,采用随机数字表法分为2组;对照组患者63例给予阿司匹林、二甲双胍和阿托伐他汀钙治疗,观察组患者63例加用利拉鲁肽注射液治疗。于治疗前、治疗后6个月分别测定血糖、血脂、炎症因子和同型半胱氨酸,比较2组患者的治疗效果及药物不良反应。结果与治疗前相比,治疗后2组患者同型半胱氨酸、空腹血糖、总胆固醇、C反应蛋白、糖化血红蛋白、餐后2h血糖、甘油三酯、肿瘤坏死因子、白细胞介素-6和低密度脂蛋白胆固醇均显著下降,高密度脂蛋白胆固醇均显著升高(P <0.05)。治疗后观察组空腹血糖、餐后2h血糖、糖化血红蛋白、同型半胱氨酸、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、C反应蛋白、白细胞介素-6、肿瘤坏死因子显著低于对照组,治疗总有效率和高密度脂蛋白胆固醇水平显著高于对照组(P <0.05)。结论利拉鲁肽治疗可有效控制血糖外,同时可降低2型糖尿病合并冠心病患者的炎症因子,降低同型半胱氨酸水平。Objective To investigate the effect of liraglutide on serum inflammatory factors and homocysteine levels in the patients with type 2 diabetics(T2DM) combined with coronary heart disease. Methods 126 patients from The Second Hospital of Shijiazhuang with types 2 diabetes in combination with coronary heart disease from October 2014 to October 2016, were randomly divided into two groups, with 63 in each group. The control group was administered with aspirin, metformin and atorvastatin calcium. Whereas in observation group, besides above mentioned tablets, liraglutide injection was applied in the treatment. Blood glucose level, blood lipid level, inflammatory factors level and and homocysteine were measured before treatment 6 months and 6 months after treatment to compare the therapeutic effects as well as adverse drug reaction between two groups. Results After treatment, all the indexes like homocysteine, fasting blood glucose, total cholesterol, C-reactive protein, glycated hemoglobin, 2h postprandial blood glucose, triglycerides, tumor necrosis factor, interleukin-6 and low density lipoprotein cholesterin all were significantly decreased, and the high-density lipoprotein cholesterol were significantly increased in control group and observation group (P 〈0.05). Compared with control group, it is significantly lower than that in observation group in the case of homocysteine, fasting blood glucose, total cholesterol, C-reactive protein, glycated hemoglobin, 2h postprandial blood glucose, triglycerides, tumor necrosis factor, interleukin-6 and low density lipoprotein cholesterin, and the total effective rate and high density lipoprotein cholesterol in the observation group is significantly better than that in control group (P 〈0.05). Conclusion Besides from tightly control of blood sugar level, liraglutide treatment can also effectively improve the inflammatory condition and decrease the level of homocysteine in patients with diabetes and coronary heart disease.
