摘 要:目的观察丹瓜方对糖尿病大鼠模型心肌相关能量代谢指标及心肌形态的影响,为痰瘀同治的丹瓜方防治糖尿病相关心肌损害提供借鉴。方法 38只正常雄性SD大鼠随机分成正常(简称NOR)组、模型(简称MOD)组、糖脉康(简称TMK)组、丹瓜方高、中、低剂量(分别简称HDG、MDG、LDG)组,干预12周后行糖耐量试验。检测心肌组织匀浆三磷酸腺苷(adenosine triphosphate,ATP)与游离脂肪酸(free fatty acid,FFA)、HE染色观察心肌形态,实时荧光定量PCR(qPCR)检测心肌组织PPAR-α,酶联免疫吸附法(ELISA)测定心肌组织匀浆过氧化物酶体增殖激活受体α(peroxisome proliferator-activated receptor-α,PPAR-α)蛋白及葡萄糖转运蛋白4(glucose transporter type 4,GLUT-4)的表达。结果与NOR组比较,MOD组FFA、PPAR-α蛋白、GLUT-4表达均降低(P<0.01,P<0.05),血糖(blood glucose,BG)明显升高(P<0.01),HE染色肌纤维纹理模糊、紊乱,心肌细胞肥大,部分变性坏死。与MOD组比较,各治疗组空腹血糖(fasting blood glucose,FBG)、FFA较低(P<0.01,P<0.05),MDG组糖耐量1 h(1 h blood sugar,1 h BG)、LDG组糖耐量2 h(2 h blood sugar,2 h BG)较低(P<0.05,P<0.01),HDG组ATP较高(P<0.05),HDG、MDG组乳酸脱氢酶(lactate dehydrogenase,LDH)较低(P<0.05,P<0.01),MDG组PPAR-α蛋白及GLUT-4表达较高(P<0.05),各治疗组HE染色均有不同程度的改善,MDG组较为明显。与TMK组比较,MDG组FBG、LDG组2h BG均更低(P<0.01);MDG、HDG组LDH较低(P<0.01)。结论丹瓜方干预对糖尿病大鼠心肌损害有治疗获益,机制之一是有效改善其能量的利用方式,痰瘀互结可能是糖尿病心肌损害的主要病机。Objective To observe the effects of Dangua Recipe (DGR) on myocardial adenosine triphosphate (ATP), peroxisome proliferator-activated receptor-α (PPAR-α), glucose transporter type 4 (GLUT-4)and other related energy metabolism indicators and myocardial morphology in diabetic rats, and to provide reference for preventing and treating diabetes related myocardial damage. Methods To- tally 38 normal male SD rats were randomly divided into the normal group, the model group, Tang Mai Kang (TMK) group, high, middle and low dose DGR groups. Glucose tolerance test was performed after 12 weeks intervention. ATP and free fatty acids (FFA) contents were detected in myocardial homogenate. Morphology of heart muscle was observed by HE staining. PPAR-α expression in myocardial tissue was detected in Real-time fluorescence quantitative PCR (qPCR), Expressions of PPAR-α protein and GLUT-4 in myocardial homogenate tissue were determined by ELISA. Results Compared with the normal group, the protein expressions of FFA, PPAR-α, and GLUT-4 all decreased (P 〈0.01, P 〈0.05), blood glucose ( BG)significantly increased (P 〈0.01 ) in the model group. Besides, HE stained muscle fiber texture was blurred and disordered. Hypertrophy of myocardial cells could be seen. And partial degenerated and necrotic cells could also be seen. Compared with the model group, fasting blood glucose( FBG)and FFA were lower in each treatment group (P 〈0.01, P 〈0.05). Sugar tolerance test 1 h blood sugar (1 h BG)in the middle dose DGR group and 2 h blood sugar (2 h BG)in the low dose DGR group were lower (P 〈0.05, P〈0.01 ). ATP was higher in the high dose DGR group (P 〈0.05). Lactate dehydrogenase (LDH) was lower in middle and high dose DGR groups (P 〈0.05, P 〈0.01 ). The expressions of PPAR-α and GLUT-4 protein were higher in the middle dose DGR group (P 〈0.05). Results of HE staining was somewhat improved in each treatment group, and the improvement was most
