摘 要:随着体外循环技术的不断发展,许多复杂的心脏手术得以开展,然而术中及术后出血仍是心脏外科手术所面临的难题,特别是术中体外循环的非生理灌注、低温或深低温等因素对血液成分的破坏,导致患者术中及术后出血增加。因此异体血输注成为心脏外科的常见现象。但越来越多的研究证实异体血输注会带来各种不良后果。随着近年来血液资源紧张,做好围手术期血液保护,减少术中及术后出血,减少或避免异体血输注受到心脏外科医生的高度重视。近年来,围手术期血液保护及减少异体血输注的研究取得了很大进展,本文就取得的研究进展予以综述。With the development of cardiopulmonary bypass technology, many complex cardiac operations were made possible. However, intraoperative and postoperative bleeding remains a challenge for cardiac surgery. Especially non-physiological perfusion, hypothermia or deep hypothermia of extracorporeal circulation to the blood can cause increased intraoperative and postoperative bleeding. Therefore, allogeneic blood transfusion is a common technique used in cardiac surgery. However, more and more studies suggest that allogeneic blood transfusion may bring a variety of adverse consequences. In recent years, due to strained blood supply, blood protection during perioperative period, reduction of intraoperative and postoperative hemorrhage, reduction or avoidance of allogeneic blood infusion are highly valued by cardiac surgeons. Meanwhile, great progress has been made in the study of perioperative blood protection and the reduction of allogeneic blood transfusion. This paper reviews the recent progress of these studies.
