化脓性链球菌毒力基因检测及模式分类分析

摘  要:目的调查60株化脓性链球菌毒力基因存在状况及毒力基因携带模式。方法 60株化脓性链球菌分离自2014年1月至2017年10月南京医科大学附属淮安第一医院与南京中医药大学附属医院急性扁桃体炎患者的病灶部位拭子标本。采用PCR及序列分析的方法分析侵袭性酶毒力基因(hyl A、spe B、endo S)、超抗原毒力基因(sme Z)、免疫逃逸毒力基因(sic)及黏附素毒力基因(prt F1)等6种毒力基因,并基于阳性检出模式进行毒力基因分类。结果侵袭性酶spe B、endo S基因和超抗原sme Z基因检出率均达100%,hyl A检出率为50%,sic检出率为23.3%,prt F1未检出。根据毒力基因检测结果,60株化脓性链球菌可分为4种类型,检出hyl A、spe B、endo S、sme Z等4种毒力基因有29株(48.3%),检出spe B、endo S、sme Z等3种毒力基因17株(28.3%),检出spe B、endo S、sme Z、sic等4种毒力基因13株(21.7%),检出hyl A、spe B、endo S、sme Z、sic等5种毒力基因1株(1.7%)。结论临床分离化脓性链球菌侵袭性酶spe B、endo S基因和超抗原sme Z基因检出率均达100%,推测其可能是化脓性链球菌感染导致感染灶局部炎症和坏死的原因之一。Objective To investigate the distribution and carrying mode of virulence genes in 60 strains of Streptococcus pyogenes( S.pyogenes). Methods Sixty strains of S. pyogenes were isolated from the throat swab specimens of the patients with acute tonsillitis in Huai'an First Hospital Affiliated to Nanjing Medical University and Affiliated Hospital of Nanjing University of Traditional Chinese Medicine during January 2014 and October 2017. Then,6 kinds of virulence genes encoding invasive enzymes( hyl A,spe B and endo S),superantigens( sme Z),immune escape protein( sic) and adhesin( prt F1) were analyzed by PCR and sequencing,and the positive virulence genes were further classified. Results The detection rates of spe B,endo S and sme Z genes in 60 strains of S. pyogenes were 100%,those of hyl A and sic were 50% and 23.3%,respectively,and that of prt F1 was zero. Based on the result of positive virulence genes,60 strains of S. pyogenes were divided into 4 genotypes,including 29( 48.3%) strains of hyl A+spe B+endo S+sme Z,17( 28.3%) strains of spe B+endo S+sme Z,13( 21.7%) strains of spe B+endo S+sme Z+sic,and 1( 1.7%) strain of hyl A+spe B+endo S+sme Z+sic. Conclusion The detection rates of spe B,endo S and sme Z in S.pyogenes isolated from clinical specimens are all 100%,indicating that they may be one of the causes leading to local inflammation and necrosis in the infection of suppurative streptococcus.
