腹腔镜下全子宫切除同时切除双侧输卵管对卵巢储备的影响

摘　　要：目的:评估腹腔镜下全子宫切除同时切除双侧输卵管对卵巢储备的影响。方法:30例18~45岁的绝经前妇女接受腹腔镜下全子宫切除术。采用双侧输卵管切除术(n=15)与非输卵管切除术(n=15)在腹腔镜子宫切除术时保留卵巢。观察术前、术后4~6周及术后3个月抗苗勒氏管激素(anti müllerian hormone,AMH),手术时间,手术出血量。结果:两组基线、术后4~6周及术后3个月AMH水平比较差异均无统计学意义(P>0.05);两组手术时间及失血量比较差异均无统计学意义(P>0.05)。结论:腹腔镜下全子宫切除术同时切除输卵管是一种安全的手术方法,它对卵巢储备没有任何短期的不良影响。ObjectiveTo examine effects of salpingectomy during laparoscopic hysterectomy on ovarian reserve.Method：Thirty premenopausal women aged 18 to 45 years underwent laparoscopic hysterectomy.Bilateral salpingectomy（n=15） and no salpingectomy（n=15） at the time of laparoscopic hysterectomy with ovarian preservation.Anti müllerian hormone（AMH） measured preoperatively,at 4 to 6 weeks postoperatively,and at 3 months postoperatively,measured operative time and estimated blood loss.Result：The mean AMH levels at baseline,4 to 6 weeks,3 months were not different significantly between two groups（P〉0.05）.No difference in operative time or estimated blood loss was observed between two groups（P〉0.05）.Conclusion：Salpingectomy at the time of laparoscopic hysterectomy with ovarian preservation is a safe procedure that does not appear to have any short-term deleterious effects on ovarian reserve.
