五味消毒饮保留灌肠对老年急性盆腔炎(湿毒壅盛证)患者血清IL-8、CRP、D-D水平及免疫学指标的影响

摘　　要：目的观察五味消毒饮保留灌肠对老年急性盆腔炎(湿毒壅盛证)患者血清白介素-8(IL-8)、C反应蛋白(CRP)、D-二聚体(D-D)水平及免疫学指标的影响。方法患者128例随机分为两组,均给予西医常规治疗,对照组给予参芎胶囊治疗,治疗组在对照组基础上联合五味消毒饮保留灌肠治疗。观察两组中医证候积分、临床疗效,同时检测患者血清IL-8、CRP、D-D水平及免疫学指标。结果治疗后两组中医证候积分均显著降低(P <0.05),且治疗组低于对照组(P <0.05);治疗组总有效率为93.75%,显著高于对照组的78.13%(P <0.05);治疗后两组血清IL-8、CRP、D-D水平均显著降低(P <0.05),且治疗组低于对照组(P <0.05);两组CD3+、CD4+、CD4+/CD8+水平均显著升高(P <0.05),且治疗组高于对照组(P <0.05),两组CD8+水平均显著降低(P <0.05),且治疗组低于对照组(P <0.05)。结论五味消毒饮保留灌肠联合参芎胶囊可有效改善老年急性盆腔炎(湿毒壅盛证)患者血清IL-8、CRP、D-D水平,提高机体免疫力和临床疗效。Objective： To observe the effects of Wuwei Xiaodu Decoction retention enema on levels of serum IL-8,CRP and D-D and immunological indices of the elderly patients with excessive accumulation of phlegm-dampness type acute pelvic inflammatory disease. Methods： 128 were randomly divided into the treatment group and the control group,64 in each. Both were given conventional treatment,on which basis,the control group was given Shenxiong Capsules,and the treatment group combined Wuwei Xiaodu Decoction retention enema on the basis of control group. The symptom scores and clinical efficacy were observed,and the serum levels of IL-8,CRP and D-D and immunological indices were detected. Results： TCM symptom scores of two groups after treatment were decreased significantly（P〈0.05）,and the treatment group was lower than the control group（P〈0.05）;the total effective rate of the treatment group（93.75%） was significantly higher than 78.13% in the control group（P〈0.05）;after treatment,the levels of serum IL-8,CRP and D-D were significantly decreased（P〈0.05）,and the treatment group was lower than the control group（P〈0.05）;levels of CD3＋,CD4＋ and CD4＋/CD8＋ after treatment were significantly increased（P〈0.05）,and the treatment group was higher than control group（P〈0.05）;the CD8＋levels decreased significantly（P〈0.05）,and the treatment group was lower than the control group（P〈0.05）.Conclusion： Wuwei Xiaodu Decoction retention enema combined with Shenxiong Capsules can effectively improve the levels of IL-8,CRP and D-D in elderly patients with excessive accumulation of phlegm-dampness type acute pelvic inflammation,and improve immunity and clinical efficacy.
