腹腔镜辅助阴式全子宫切除术治疗巨大子宫肌瘤临床研究

摘　　要：目的研究巨大子宫肌瘤患者通过腹腔镜辅助阴式全子宫治疗的治疗效果。方法对该院在2017年1月—2018年1月,这一期间内收治的患者随机抽取出64例患者,把他们当成该次研究的研究目标,根据患者的例数将小组进行划分,分为观察组(n=32)和对照组(n=32)。观察组患者通过腹腔镜辅助阴式全子宫切除手术进行治疗,对照组患者通过开腹子宫切除进行治疗。将两组患者所获得的治疗成功率以及手术指标进行比较。结果观察组患者所获得的治疗成功率为100.0%,对照组的治疗成功率为84.4%(χ~2=10.259 4,P<0.05),以及手术指标(手术时间、住院时间、术中出血量、导尿管的留置时间)分别为:(100.3±20.1)min,(3.5±1.6)d,(91.6±30.2)mL,(23.5±8.5)h,对照组的手术指标:(59.8±9.1)min,(7.3±2.1)d,(101.2±26.9)mL,(49.2±15.7)h,(t=15.264 8、6.295 4、16.2487、 10.624 3,P<0.05)。结论通过腹腔镜辅助阴式全子宫切除术对巨大子宫肌瘤患者进行治疗,治疗有效率高、伤口较小、安全性高、手术指标优良,值得在临床上推广以及应用。Objective To study the effect of laparoscopic assisted vaginal hysterectomy in treatment of giant hys- teromyoma. Methods 64 cases of patients in our hospital from January 2017 to January 2018 were selected and divided into two groups with 32 cases in each, the observation group were treated with laparoscopic assisted vaginal hysterectomy, while the control group were treated with open hysterectomy, and the treatment success rate and operative indicators were compared between the two groups. Results The treatment success rate in the observation group and in the control group was respectively 100.09o and 84.49o （χ^2=10.259 4,P〈0.05）, and the operation indicators such as operation time, length of stay, intraoperative bleeding amount, and catheter indwelling time in the observation group and in the control group were respectively （ 100.3±20.1 ）rain, （3.5±1.6）d, （91.6±30.2）mL, （23.5±8.5）h and （59.8±9.1）rain, （7.3±2.1）d,（101.2±26.9）mL, （49.2±15.7）min,（t=15.264 8, 6.295 4, 16.248 7, 10.624 3, P〈0.05）. Conclusion The effective rate of laparoscopic assisted vaginal hysterectomy in treatment of giant hysteromyoma is high with small wound, high safety and good operative indicators, and it is worth clinical promotion and application.
