经尿道等离子柱状电极整块切除术应用于非肌层浸润性膀胱癌治疗中疗效分析

摘　　要：目的探讨经尿道等离子柱状电极整块切除术治疗非肌层浸润性膀胱癌的临床效果,为临床应用提供参考依据。方法方便选取该院2015年1月—2017年1月所收治非肌层浸润性膀胱癌手术患者102例,根据术式不同分为研究组和对照组。其中,对照组51例,予以经尿道膀胱肿瘤电切术进行治疗。研究组51例,予以经尿道等离子柱状电极整块切除术进行治疗。比较两组临床表现指标差异,并随访12个月,记录两组并发症发生情况及复发情况。结果两组患者各项临床指标相比较,差异无统计学意义(t=0.991、0.271、0.496、1.088,P>0.01)。研究组患者中,仅出现1例膀胱穿孔、术中未出现闭孔神经反射,其并发症发生率为1.92%,明显低于对照组的23.53%,差异有统计学意义(χ~2=8.816,P<0.01);研究组复发率为37.25%,与对照组的41.18%相比较,差异无统计学意义(χ~2=0.041,P>0.05)。结论经尿道等离子柱状电极整块切除术应用于非肌层浸润性膀胱癌治疗中,术中操作娴熟,术后表现好,且并发症更少,复发率维持在较低水平,其整体表现安全可靠,可作为成熟术式推荐临床推广应用。Objective To investigate the clinical effect of transurethral plasma column resection for non-muscle invasive bladder cancer, and to provide reference for clinical application. Methods A total of 102 patients with non-muscle invasive bladder cancer admitted to the hospital from January 2015 to January 2017 were convenient selected and enrolled. The patients were divided into study group and control group according to different surgical procedures. Among them, 51 patients in the control group were treated with transurethral resection of bladder tumor. A total of 51 patients in the study group were treated with a urethral plasma column electrode resection. The differences of clinical manifestations between the two groups were compared and followed up for 12 months. The complications and recurrence of the two groups were recorded. Results The clinical parameters of the two groups were compared, and the difference was not statistically significant （t= 0.991, 0.271, 0.496, 1.088,P〉0.01）. Among the patients in the study group, only one ease of bladder perforation occurred, and no obturator nerve reflex occurred during operation. The complication rate was 1.92%, which was significantly lower than that of the control group 23.53%, the difference was statistically significant （X==8.816,P〈0.01）; The recurrence rate of the study group was 37.25%, compared with 41.18% of the control group. The difference was not statistically significant（x== 0.041 ,P〉0.05）. Conclusion Transurethral plasma column electrode resection is used in the treatment of non-muscle inva- sire bladder cancer. The operation is skillful, the postoperative performance is good, and the complications are less. The re-currence rate is maintained at a low level, safe and reliable, it can be used as a mature surgical recommendation for clinical promotion.
