中山市4508例女性宫颈细胞HPV基因分型分析

摘　　要：目的分析中山市人民医院妇科门诊和常规体检妇女的人乳头状瘤病毒(HPV)的感染状况及其基因分型特点,为该地区宫颈癌的防治提供依据。方法收集2015年6月-2017年12月在中山市人民医院妇科门诊和常规体检的4 508例妇女的宫颈脱落细胞标本,采用PCR+导流杂交法对标本进行HPV基因分型检测,其中包括15种高危型HPV(HPV-16、18、31、33、35、39、45、51、52、53、56、58、59、66、68)和6种低危型HPV (HPV-6、11、42、43、44、CP8304)。对HPV感染率、亚型分布及年龄特点进行总结分析。结果共检出HPV阳性1 668例,总感染率为37.00%(1 668/4 508),单一型别感染占总感染的79.80%(1 331/1 668),多重型别感染占20.20%(337/1 668),大于或等于60岁的感染率最高,感染率为59.07%(114/193)。感染率排前三分别为52亚型,检出率为9.38%(423/4 508);16亚型检出率为6.61%(298/4 508);58亚型检出率为5.86%(264/4 508)。结论中山市妇女HPV感染率高,HPV最高的基因亚型是52型,多数为单一感染,60岁以上人群检出率最高。Objective To analyze the infection status and genotyping characteristics of human papillomavirus （HPV） from gynecological outpatient and routine physical examination women in Zhongshan City People＇s Hospital. To provide evidence for the prevention and treatment of cervical cancer in the region. Methods 4508 samples of cervical exfoliated cells were collected from gynecological outpatient department and routine physical examination in Zhongshan City People＇s Hospital from June 2015 to December 2017. The HPV genotyping was detected by PCR＋ conductivity hybridization, including 15 types of high-risk HPV （HPV-16, 18, 31, 33, 35, 39, 45, 52, 53, 56, 58, 59, 66, 68） and six types of low-risk HPV （HPV- 6, 11, 42, 43, 44, CP8304）. The infection rate, subtype distribution and age character of HPV were summarized and analyzed. Results The total infection rate was 37.00% （1668/4508）. The single type of infection accounted for 68.05% of the total infection （1135/1668）, and the multiple type of infection accounted for 31.95% （533/1668）. The infection rate was highest among those aged 60 years or older, with an infection rate of 59.07% （114/193）. The top three of infection rates were the 52 subtype, the 16 subtype, and the 58 subtype, with the positive rate of 9.38% （423/4508）, 6.61% （298/4508）, and 5.86% （264/4508）, respectively. Conclusion There are regional differences in HPV genotype infection. The most common subtype of HPV is the type 52, most of which are single infections. The highest detection rate is found among people over 60 years old.
