院前急救185例脑卒中患者的流行病学特点分析

摘　　要：目的了解院前急救脑卒中患者的流行病学特征,为脑卒中的预防和急诊救治提供依据。方法收集医院急诊科院前急救的185例脑卒中患者的临床资料,按脑卒中患者年龄、发病季节、就诊时间分布情况进行回顾性分析。结果 185例患者中男121例,女64例,年龄17～94岁,70～79岁年龄组患者最多,占37. 30%(69/185);缺血性脑卒中109(58. 92%)例,出血性脑卒中76(41. 08%)例;发病季节分布以冬季发病较多,占31. 89%(59/185);就诊时间<4 h的患者133(71. 89%)例,4～6 h的患者20(10. 81%)例,> 6 h的患者32(17. 30%)例。结论需加强人群脑卒中知识的健康教育,纠正不健康的生活方式,积极控制危险因素。同时建立脑卒中急救专业化队伍,实现脑卒中诊疗的院前和院内一体化,保障脑卒中急救绿色通道的通畅。Objective To investigate the clinical and epidemiological features of stroke patients with pre-hospital emergency treatment in a certain hospital in Qinzhou, and to provide reference forstroke prevention and emergency treatment. Methods The clinical data of 185 stroke patients with pre-hospital emergency treatment were retrospectively analyzed according to the age distribution,season of onset and time distribution of stroke patients. Results There were 185 stroke patients with pre-hospital emergency treatment, including 121 males and 64 females. The age of patients ranged from 17 years old to 94 years old, and most of the patients were in 70-79 years age group. There were 109 （58.92%） cases of ischemic stroke and 76 cases （41.08%） of hemorrhagic stroke. In terms of seasonal distribution, about 31.89 %（ 59/185） cases occurred in winter. There were 133（71.89%） patients with pre-hospital time less than 4 hours, 20（10.81%） patients with pre-hospital time between 4 to 6 hours and 32（17.30%） patients with pre-hospital time over 6 hours. Conclusion More efforts should be paid to strengthen the health education of stroke-related knowledge,correct the unhealthy behaviors and actively control the risk factors. It is also required to establish specialized first-aid teams for stroke patients, integrate the pre-hospital and in-hospital treatment, and ensure the green channel process of stroke rescue.
