摘 要:目的观察清下活血方对重症急性胰腺炎大鼠小肠黏膜中IL-6mRNA、IL-10mRNA及黏蛋白MUC2表达水平的影响,探讨清下活血方对重症急性胰腺炎大鼠肠黏膜屏障损伤的保护作用机制。方法将72只SD大鼠随机分为假手术组、模型组和中药组,每组24只。模型组和中药组大鼠采用胰胆管逆行注射5%牛磺胆酸钠方法建立重症急性胰腺炎模型,建模后2 h中药组给予清下活血方灌胃,假手术组和模型组给予生理盐水灌胃,均每12 h 1次,灌胃3次。分别于灌胃后24,48,72 h取血检测3组血清淀粉酶水平,观察小肠黏膜病理改变,检测小肠黏膜组织中IL-6mRNA、IL-10mRNA和MUC2表达水平。结果模型组、中药组各时间点血清淀粉酶水平和小肠黏膜组织中IL-6mRNA、IL-10mRNA表达水平均明显高于假手术组(P均<0. 05);中药组各时间点血清淀粉酶水平和小肠黏膜组织中IL-6mRNA水平均明显低于模型组(P均<0. 05),而IL-10mRNA表达水平均明显高于模型组(P均<0. 05);中药组各时间点小肠黏膜损伤指数评分均明显低于模型组(P均<0. 05);模型组各时间点小肠黏膜组织中MUC2表达水平均明显低于假手术组(P均<0. 05),而中药组各时间点MUC2表达水平均明显高于模型组(P均<0. 05)。结论清下活血方通过重建促炎和抗炎细胞因子的平衡,增加MUC2分泌量,减轻肠道组织损伤而起到对肠黏膜屏障的保护作用。Objective It is to observe the effects of Qingxia Huoxue Decoction on the expression of IL-6 mRNA, IL-10 mRNA and mucin MUC2 in small intestinal mucosa of rats with severe acute pancreatitis, and to investigate the protecting mechanism of this decoction on the damage of intestinal mucosal barrier of severe acute pancteatitis rats. Methods Seventy-two rattus norregicus rats were randomly divided into sham operation group, model group and Chinese medicine group, with 24 rats in each group. In the model group and the Chinese medicine group, the severe acute pancteatitis models were established by retrograde injection of 5% sodium taurocholate into the pancreaticobiliary duct. After 2 hours of modeling, the Chinese medicine group was given Qingxia Huoxue Decoction by gavage, and the sham operation group and the model group were given normal saline by gavage. They were treated once every 12 hours and 3 times. Serum amylase levels were measured at 24, 48, and 72 h after gavage, and the pathological changes of small intestinal mucosa were observed. The expressions of IL-6 mRNA, IL-10 mRNA and MUC2 in small intestinal mucosa were detected. Results The levels of serum amylase and IL-6 mRNA and IL-10 mRNA in the small intestinal mucosa of the model group and the Chinese medicine group were significantly higher than those in the sham operation group ( P 〈0.05). The serum amylase level in the Chinese medicine group at each time point and the level of IL-6 mRNA in the small intestinal mucosa was significantly lower than that in the model group ( P 〈0.05), while the IL-10 mRNA expression level was significantly higher than that in the model group ( P 〈0.05). The small intestinal mucosal injury index in the Chinese medicine group at each time point was significantly lower than that in the model group ( P 〈0.05).
