肺肾同治方治疗儿童过敏性紫癜性肾炎疗效观察

摘　　要：目的探讨肺肾同治方治疗儿童过敏性紫癜性肾炎的疗效及对血清胱抑素C(Cys C)、血浆内皮素-1(ET-1)的影响。方法将90例过敏性紫癜性肾炎患儿随机分为治疗组和对照组,每组45例。对照组给予西医常规治疗,治疗组在对照组的基础上加服肺肾同治方,2组疗程均为12周。观察2组治疗前后24 h尿蛋白定量、尿红细胞计数、血清Cys C、血浆ET-1水平变化情况,统计2组临床疗效、不良反应及复发情况。结果治疗后2组24 h尿蛋白定量、尿红细胞计数、血清Cys C、血浆ET-1水平均显著降低(P均<0. 05),且治疗组各项指标改善情况均明显优于对照组(P均<0. 05);治疗组总有效率显著高于对照组(P <0. 05),不良反应发生率及复发率均明显低于对照组(P均<0. 05)。结论肺肾同治方治疗儿童过敏性紫癜性肾炎疗效显著,降低了血清Cys C和血浆ET-1水平,促进了肾损害的恢复,且不良反应少,复发率低。Objective It is to investigate the effect of Feishen Tongzhi Decoction on children with allergic purpuric nephritis and the effects of serum cystatin C （CysC） and plasma endothelin-1 （ET-1）. Methods Ninety children with allergic purpuric nephritis were randomly divided into treatment group and control group, with 45 cases in each group. The control group was given conventional treatment with western medicine. On the basis of the control group, the treatment group was treated with Tongshen Tongzhi decoction. Both groups were treated for 12 weeks. The changes of urine protein quantitation, urinary red blood cell count, serum CysC and plasma ET-1 were observed before and after treatment in the two groups. The clinical efficacy, adverse reaction and recurrence of the two groups were statistically analyzed. Results The urine protein quantitation, urinary red blood cell count, levels of serum CysC and plasma ET-1 were significantly decreased in the two groups after treatment （ P 〈0.05）, and the improvements of the treatment group were significantly better than those in the control group （ P 〈0.05）. The total effective rate of the treatment group was significantly higher than that of the control group （ P 〈0.05）, and the incidence of adverse reactions and recurrence rate were significantly lower than that of the control group （ P 〈0.05）. Conclusion Feishentongzhi Decoction has significant curative effect on children with allergic purpuric nephritis, it could decrease the levels of serum CysC, plasma ET-1, promote recovery of renal damage, and has fewer adverse reactions and lower recurrence rate.
