摘　　要：目的探讨^(99m)Tc标记的甲氧基异丁基异腈(^(99m)Tc-MIBI)门控单光子发射计算机断层扫描(SPECT/CT)心肌灌注显像评估2型糖尿病患者左心室功能的价值。方法选取自2013年1月至2016年12月在北部战区总医院核医学科行门控负荷/静息心肌灌注显像的62例2型糖尿病患者为糖尿病组,另选取同期在我院进行健康体检的志愿者50例为健康组。两组受试者均行^(99m)Tc-MIBI门控SPECT/CT心肌灌注显像,并由两名核医学医师进行视觉分析,应用17节段法进行半定量分析。比较两组受试者的总负荷评分(SSS)、总静息评分(SRS)、总差值(SDS)、室壁增厚总积分(STS)和室壁运动总积分(SMS),以及左心室功能参数,包括左室射血分数(LVEF)、舒张末期容积(EDV)、收缩末期容积(ESV)、高峰充盈率(PFR)和高峰射血率(PER)。结果糖尿病组SSS和SDS评分高于健康组,组间比较,差异有统计学意义(P<0.05)。两组SRS、SMS及STS评分比较,差异无统计学意义(P>0.05)。糖尿病组静息LVEF、负荷LVEF、PFR低于健康组,EDV高于健康组,组间比较,差异有统计学意义(P<0.05)。两组ESV、PER比较,差异无统计学意义(P>0.05)。结论 ^(99m)Tc-MIBI门控SPECT/CT心肌灌注显像可用于评估2型糖尿病患者左心室功能变化,有助于糖尿病患者心肌损害的早期诊断并指导治疗决策。Objective To investigate the changes of left ventricular function in patients with type 2 diabetes mellitus by ^99mTc-MIBI gated SPECT/CT myocardial perfusion imaging.Methods A retrospective study was performed on 62 cases of patients with type 2 diabetes mellitus by ^99mTc-MIBI gated SPECT/CT myocardial perfusion imaging（diabetic group）who were admitted from January 2013 to December 2016,and another 50 cases healthy volunteers were in the healthy group.In both groups, ^99mTc-MIBI gated SPECT/CT myocardial perfusion imaging was performed,and visual analysis was performed by two nuclear physicians,and semi-quantitative analysis was performed by 17 segmental methods.Comparison of two groups of cases with total load rating（SSS）,total resting score（SRS）,total difference（SDS）,ventricular wall thickening（STS）,total integral ventricular wall motion score（SMS）,left ventricular function parameters,including left entricular ejection fraction（LVEF）,end-diastolic volume（EDV）,end systolic volume（ESV）,peak filling rate（PFR）and peak ejection rate（PER）.Results The SSS and SDS scores of the diabetic group were higher than those of the healthy group,and the difference was statistically significant（ P 〈0.05）.SRS,SMS and STS scores were compared between the two groups,and the differences were not statistically significant（ P 〉0.05）.In the diabetic group,resting LVEF,loading LVEF and PFR were lower than those in the healthy group and EDV was higher than those in the healthy group,and the difference was statistically significant （ P 〈 0.05）.Comparison of ESV and PER between the two groups showed no significant difference（ P 〉0.05）.Conclusion ^99mTc-MIBI gated SPECT/CT myocardial perfusion imaging can be used to evaluate left ventricular function in patients with type 2 diabetes mellitus,and it is helpful for the early diagnosis and treatment decisions of cardiac damage in patients with diabetes mellitus.
