摘  要:目的探讨^(18)F-氟脱氧葡萄糖(^(18)F-FDG)正电子发射计算机断层成像(PET)行心肌代谢显像时不同血糖调节方法对其显像成功率的影响。方法选取自2013年8月至2018年4月收治的拟诊为心肌缺血的患者83例为研究对象。对拟行^(18)F-FDG PET心肌代谢显像的受检者根据血糖水平给予葡萄糖负荷,如果受检者血糖值>8.8 mmol/L,则注射胰岛素调整血糖值至7.9~8.8 mmol/L,纳入A组;如果血糖值为7.9~8.8 mmol/L,则纳入B组。比较A、B两组患者的显像成功率。结果进行血糖调节后,A组显像成功41例,显像失败3例,显像成功率为93.2%(41/44)。B组显像成功29例,显像失败10例,显像成功率为74.4%(29/39)。A组显像成功率显著高于B组,两组比较,差异有统计学意义(P<0.05)。结论 ^(18)F-FDG PET心肌代谢显像时,应使用先葡萄糖负荷后胰岛素调整的方法进行血糖调节,可获得更好的图像。Objective To investigate the effects of different blood glucose regulation methods on the success rate of myocardial metabolic imaging in ^18F-FDG PET.Methods A retrospective study was performed on 83 cases of patients with myocardial ischemia who were admitted from August 2013 to April 2018.Patients who underwent ^18F-FDG PET myocardial metabolic imaging were given glucose load according to their blood glucose level.If the blood glucose level of subjects was more than 8.8 mmol/L,insulin was injected to adjust the blood glucose level to 7.9-8.8 mmol/L,which was included in Group A.If the blood glucose level is 7.9-8.8 mmol/L,which was included in Group B.The success rate of imaging was compared between Group A and Group B.Results After the adjustment of blood glucose,41 cases of successful imaging and 3 cases of failure of imaging were successfully performed in Group A,with success rate of 93.2%(41/44).In Group B,there were 29 successful cases and 10 failed cases,and the success rate was 74.4%(29/39).The success rate of imaging in Group A was significantly higher than that in Group B,and the difference was statistically significant ( P 〈0.05).Conclusion ^18F-FDG PET myocardial metabolic imaging,glucose adjustment should be used after the glucose adjustment method for blood glucose regulation to get better images.
