补肾活血汤联合阿司匹林对复发性流产患者子宫动脉血流的影响

摘　　要：目的旨在研究补肾活血汤联合阿司匹林对子宫动脉血流异常的复发性流产的临床疗效。方法将60例符合复发性流产诊断标准、纳入标准的子宫动脉血流阻力异常的早孕患者随机分为治疗组和对照组。治疗组以中药联合阿司匹林治疗,对照组单纯使用阿司匹林治疗。观察两组患者治疗前(孕6周)及治疗后孕8周、孕10周的彩色多普勒测定的子宫动脉血流指数(RI、S/D)、不良反应。结果治疗组患者临床有效率明显高于对照组(P<0.05),RI、S/D之和指标改善情况均优于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。两组RI、S/D之和随孕周增加均降低。治疗组孕8周RI、S/D之和与孕6周比较有显著性差异(P<0.05),对照组孕8周左RI、S/D之和与孕6周比较无显著性差异(P>0.05)。结论补肾活血汤联合阿司匹林治疗子宫动脉血流阻力异常临床疗效显著,能改善RI、S/D,且不良反应发生率低,较单纯使用阿司匹林,可缩短治疗时间,有助于提高妊娠结局,提高胎儿存活率。Objective To study the clinical effect of Bushen Huoxue decoction combined with aspirin on uterine arteryblood flow of recurrent abortion. Methods 60 early pregnancy patients with abnormal uterine artery blood flow resistance who met the diagnostic criteria and admission criteria for recurrent abortion were randomly divided into treatmentgroup and control group. The treatment group was treated with Chinese medicine combined with aspirin, and the controlgroup was treated with aspirin alone. Uterine artery blood flow index （RI, S/D） and adverse reactions were measured bycolor Doppler measurement before treatment （6 weeks gestation） and 8 weeks and 10 weeks gestation after treatment.Results The clinical effective rate of the treatment group was significantly higher than that of the control group（P〈0.05）.The improvement of RI and sum of S/D in the observation group was better than that of the control group （P〈0.05）. Theincidence of adverse reactions in the observation group was lower than that of the control group （P〈0.05）. The RI andsum of S/D in both groups decreased with increasing gestational age. There were statistical differences in RI, sum of S/Dbetween 8 weeks of pregnancy and 6 weeks of pregnancy in the observation group（P〈0.05）. There was no statistical difference in the RI, sum of S/D between 8 weeks of pregnancy and 6 weeks of pregnancy in the control group （P〉0.05）.Conclusion Bushen Huoxue decoction combined with aspirin has significant clinical efficacy in the treatment of uterineartery blood flow resistance abnormalities, which can improve RI and S/D, with lower incidence of adverse reactions.Compared with the simple use of aspirin, it can shorten the treatment time and help to improve pregnancy outcome andfetal survival rate.
