摘 要:目的老年慢性心力衰竭患者中进行β1、β3肾上腺素能受体自身抗体的检测,分析与恶性心律失常的相关性。方法连续纳入2012年至2016年入院的老年慢性心力衰竭患者305例,以24h动态心电图结果分为恶性心律失常组(113例)和非恶性心律失常组(192例),统计两组β1-AA、β3-AA水平,并logistic回归分析恶性心律失常的危险因素。结果 (1)恶性心律失常组β1-AA、β3-AA均高于对照组,P<0.05。(2)左室射血分数、NT-proBNP、β3-AA为慢性心力衰竭恶性心律失常的相关因素(P<0.05)。结论β3肾上腺素受体自身抗体可能参与了恶性心律失常的发生的病理生理过程。Objective to detect beta 1 and beta 3 adrenergic receptor autoantibodies of elderly chronic heart failure patients, and analyze its correlation with malignant arrhythmia. Methods include 305 cases elderly chronic heart failure patients in our hospital from 2012 to 2016 continuously, based on results of dynamic 24h ECG, divide them into malignant arrhythmia group(113 cases) and non malignant arrhythmia group(192 cases), carry on statistics of beta 1-AA and beta 3-AA level of two groups, and analyze risk factors of malignant arrhythmia by logistic regression. Results(1) beta 1-AA and beta 3-AA level of malignant arrhythmia group was higher than control group, P〈0.05.(2) Left ventricular ejection fraction, NT-pro BNP, and beta 3-AA was related factors of malignant arrhythmia in chronic heart failure,(P〈0.05). Conclusion beta 3 adrenergic receptor autoantibody may be involved in pathophysiological process of occurrence of malignant arrhythmia.
