摘 要:目的:观察耳炎清滴耳液对慢性化脓性中耳炎SD大鼠中耳黏膜铜绿假单胞菌生物膜的清除效果。方法:将45只SD大鼠随机分为模型组(A)、西药组(B)、耳炎清组(C)共3组,每组15只。采用鼓室注射给药进行造模,左耳注射无菌生理盐水,右耳注射铜绿假单胞菌液,注射量均为每日0.1mL/侧。造模成功,BBF形成后,三组左鼓室均注射等量生理盐水,右鼓室A组注射生理盐水、B组注射头孢他啶和环丙沙星混合液、C组注射耳炎清滴耳液。分别在给药后的第3、7、15天,每组处死大鼠5只/次,取两侧听泡进行电镜扫描,分析中耳黏膜表面细菌生物膜覆盖率。结果:第3、7、15天3个时间点为界,各组大鼠左耳(对照耳)BBF覆盖率均为0,差异无统计学意义(P均> 0.05)。A、B、C三组大鼠右耳(模型耳)BBF覆盖率比较:A组3个时间点比较差异无统计学意义(P均> 0.05);B组和C组在3个时间点比较均呈逐渐递减的下降趋势;第7、15天2个时间点,A组与B组、A组与C组比较,差异显著(P <0.01),B组与C组比较,差异无统计学意义(P>0.05)。结论:耳炎清滴耳液对铜绿假单胞菌所致的大鼠中耳黏膜细菌生物膜有明显清除作用,其清除作用在用药7天以后显现,并可维持效果至第15天,且效果不亚于头孢他啶和环丙沙星混合液,为中医药临床治疗CSOM提供了实验依据。Objection:To observe the eliminating effects of the Eryanqing ear drops ( 耳炎清滴耳液 ) on pseudomonas aeruginosa biofilms in middle ear mucosa of SD rats with chronic suppurative otitis media. Methods: 45 rats were randomly divided into the model group ( group A), the western medicine group (group B) and Eryanqing group (group C), 15 cases in each.45 rats were given tympanum injection to make the mold. The left ear was injected with sterile saline; the right ear was injected with pseudomonas aernginosa, 0. 1mL/side daily. After the model was successful and BBF was formed, the left tympanums in groups were given equal normal saline; the right tympanums were given normal saline in group A, ceftazidime and ciprofloxacin in group B, and the Eryanqing ear drops in group C. In the 3rd, 7th, 15th day after using medcine, 5 rats in groups were killed. According to otocyst by scanning electro microscope, coverage of bacterial biofilm on middle ear mucosal surface was analyzed. Results: In the 3rd, 7th, 15th day, BBF coverage of left ear were 0%, with no significant difference (P〉0.05). BBF coverage of right ear in group A shows no significant difference (P〉0.05); that in group B and group C were decreased in the 3rd, 7th, 15th day. In 7th, 15th day, there were statistically significant difference in BBF coveragebetween group A and group B or between group A and group C (P〈0.01); there was no statistically significant difference between group B and group C (P〉0.05). Conclusion: The Eryanqing ear drops shows an obvious scavenging effect on bacterial biofilm of middle ear mucosa of rats induced by pseudomonas aernginosa. The scavenging effect was observed after 7 days and can maintain the effect until the 15th day. The effect was no less than the mixture of ceftazidime and ciprofloxacin. It provides experimental basis for treating CSOM in TCM.
