摘 要:目的分析447例疑似ABO新生儿溶血病(ABO-HDN)血清学试验结果,为临床诊断和治疗HDN提供实验室依据.方法选取2015年7月至2017年8月在昆明医科大学第二附属医院送检的怀疑为ABO-HDN的447例标本进行血型鉴定和新生儿溶血3项试验检测,探讨HDN阳性率与血型、性别、送检日龄的关系.结果447例送检标本中310例证实为ABO-HDN,阳性率为69.4%.447例疑似标本中溶血3项试验A型的阳性检出率显著高于B型(P<0.05);不同性别的患儿溶血3项试验阳性检出率基本一致,差异无统计学意义(P>0.05);患儿出生0~3 d和4~7 d送检标本溶血3项试验阳性检出率均高于>7 d的患儿,差异有统计学意义(P<0.05).结论血清学检测有助于ABO-HDN的早期诊断和治疗,新生儿血型和标本送检时间对试验结果有明显的影响,新生儿性别对结果无影响.Objective To analyze the serological test results in 447 cases of suspected neonatal ABO hemolytic disease(ABO-HDN)and to provide the laboratory evidence for clinical diagnosis and treatment of HDN.Methods The blood samples of 477 cases that had been suspected to ABO-HDN from July 2015 to August 2017 in the 2 nd Affiliated Hospital of Kunming Medical University were selected for blood type and three hemolysis test with a purpose to investigate the relationship between positive rate of HDN with the related factors such as blood type, gender, the specimens collecting time. Results There were 310 cases of ABO-HDN in the total447 blood samples with the incidence 69.4%. Detection three hemolysis test positive rate of type A was significantly higher than that of type B(P 〈 0.05). The three hemolytic test positive rate of gender was basically consistent and the statistics showed that the difference was not significant(P 〉 0.05). The positive rates of three hemolysis test of newborns among 0-3 d and 4-7 d were higher than 〉7 d, and the difference of birth was statistically significant(P 〈 0.05). Conclusion Serum test contributes to the early diagnosis and treatmenr of ABO-HDN, and the blood type of newborns and sample submission time have the obvious effect on the experimental results and there is no effect on the gender of newborns.
