趣味功能训练在前交叉韧带重建术后康复的应用

摘　　要：目的探讨趣味功能训练在前交叉韧带(ACL)重建术后康复中的临床应用价值.方法选取2015年12月至2017年6月期间收治的80例ACL损伤患者为研究对象,以随机数字表法将其分为观察组和对照组,各40例;均由同一组医生实施ACL重建术,对照组术后给予常规康复训练,观察组在常规训练基础上术后第3d增加趣味功能训练.入院时、术后2个月评估膝关节功能,进行IsoMed 2000型等速肌力测试,并采用国际膝关节文献委员会膝关节(IKDC)分级评定活动水平恢复情况.结果 2组入院时膝功能各项Lysholm评分及总分接近,差异无统计学意义(P>0.05),术后2个月各项Lysholm评分及总分均明显升高,其中观察组交锁、关节不稳定、爬楼梯、下蹲4项评分及总分高于对照组,差异有统计学意义(P<0.05).2组入院时主动运动膝关节设定角度15°、30°及45°与患者实际角度间差值的绝对值接近,等速肌力测试结果差异无统计学意义(P>0.05),术后2个月设定角度15°、30°及45°时绝对值明显降低,等速肌力明显改善,但观察组设定角度15°、30°及45°时绝对值降低幅度大于对照组,等速肌力比较有统计学意义(P<0.05).术后2个月观察组IKDC分级以A级、B级为主,对照组以B级、C级为主,2组IKDC分级分布比较有统计学意义(P<0.05).结论 ACL重建术后康复训练中应用趣味功能训练有助于尽早改善患者膝关节功能、等速肌力及活动能力.Objective To analyze the clinical value of interesting functional training in rehabilitation after anterior cruciate ligament（ACL） reconstruction. Methods 80 patients who were admitted to the hospital with ACL injury from December 2015 to June 2017 were selected as the subjects. They were divided into the treatment group and the control group by random number table method,40 cases in each group. The ACL reconstruction was performed by the same group of doctors.The control group was given routine rehabilitation training after the operation, while the treatment group was given interesting functional training based on routine training on the3 rd day after the operation. The knee function was evaluated at admission and 2 months after the operation. The isokinetic muscle strength test of IsoMed 2000 was performed.The International Knee Documentation Committee（IKDC）classification was used to evaluate the recovery of activity.Results The Lysholm knee function scores and total score of the two groups were close at admission（P 〉 0.05）. 2 months after the operation,Lysholm scores and the total score were significantly increased, and scores of interlocking, joint instability,stair climbing and squat down and the total score of the treatment group were higher than those of the control group（P 〈 0.05）. At admission,the absolute values of dif ference values between setting angles 15 degrees, 30 degrees and 45 degrees and the actual angles in the two groups were close（P 〉 0.05）. 2 months after the operation, the absolute values at 15 degrees, 30 degrees and 45 degrees decreased,and the isokinetic muscle strength were improved. However,the decrease of absolute value in the treatment group was significantly greater than that in the control group（P 〈 0.05）. 2 months after the operation, the IKDC grade of the treatment group mainly was grade A or grade B, while the grade of control group mainly was grade B or grade C（P 〈 0.05）.Conclusion The application of interesting functional training in reh
