摘 要:目的探讨纤维支气管镜(fiberoptic bronchoscope,FOB)定位方法在胸外科手术双腔支气管导管插管定位的有效性,可靠性,安全性.方法根据定位方法将160例开胸单肺通气手术病例分为F组(纤维支气管镜定位组)与T组(听诊定位组),比较两组在不同体位下单肺通气满意的例数、满意率、术中因单肺通气不满意需FOB调整定位的例数、单肺通气后45 min及90 min SpO_2值小于90%的例数.结果 F组与T组平卧位、侧卧位单肺通气满意的例数分别为76例、72例与56例、51例;平卧位、侧卧位总满意率分别为95%、90%与70%、63.8%;术中需FOB调整定位的例数分别为6例与25例;F组与T组单肺通气后45min及90 min SpO_2值小于90%的例数分别为:1例、3例与12例、23例.结论纤维支气管镜定位下双腔支气管导管插管是一种准确、可靠、安全的双腔导管插管方法,值得推广.Objective To investigate the validity,reliability and Security of fiberoptic bronchoscope location double-lumen endobronchial tubes in thoracic surgery.Methods 160 patients were randomly selected and divided into fiberoptic bronchoscope(FOB)group and auscultation(T)group. To compared the two groups in different positions under one-lung ventilation, the satisfaction cases, satisfaction rate, the numbers of FOB adjusted position in operation and the cases of SpO2 less than 90% after 45 min and 90 min. Results To compare the satisfaction cases of group F and group T with one-lung ventilation in supine and lateral position were 76 cases and72 cases and 56 cases and 51 cases,respectively,The satisfaction rate of the supine and lateral position were95%,90% and 70%,63.8%. The cases of FOB adjusted position were 6 cases and 25 cases.The number of SpO2 less than 90% after 45 min and 90 min were 1 case and 3 cases and 12 cases and 23 cases in one lung ventilation, respectively.Conclusion Fiberoptic bronchoscope locate double-lumen endobronchial tubes during thoracic surgery is an accurate,reliable and safety technology,which is worth to popularize.
