摘　　要：目的探讨经皮肾通道铥激光治疗Bosniak Ⅰ和Ⅱ期肾囊肿的的疗效及可行性。方法对2014年1月至2018年3月来自昆明医科大学第一附属医院泌尿外科54例诊断为Bosniak I或II期肾囊肿患者行经皮肾通道铥激光切除或切开囊壁治疗;回顾性分析患者的临床信息包括囊肿位置、大小等信息,手术时间,出血情况,术中及术后并发症情况及随访资料,并与50例同期运用传统腹腔镜下肾囊肿去顶减压术的患者临床资料比较。结果所有患者均获手术成功,本组经皮肾通道铥激光术式治疗的患者术中无较多出血,无肾实质或邻近脏器损伤;术后无高热、无尿路感染、无尿漏等。术后随访3～12个月,经皮肾通道铥激光术式组与传统腹腔镜组术式比较手术时间、术后住院日及术中术后并发症发生并无显著差异(P> 0.05)。结论经皮肾通道铥激光治疗Bosniak I和II期肾囊肿是一种安全、可行并有效的技术。Objective To investigate the efficacy and feasibility of percutaneous nephrolithotomy in thetreatment of Bosniak stage I or II cysts. Methods Peripheral renal access thulium laser was performed in 54 casesdiagnosed as Bosniak stage I or II cysts from January 2014 to March 2018. We retrospectively analyzed the clinicaldataincluding thelocation and size of cyst,operation time,hemorrhage situation, intraoperative and postoperativecomplications and follow-up information, and compared with the clinical data of 50 patients with traditionallaparoscopic renal cyst decompression.Results All the patients were operated successfully. There was no bleedingduring operation,no renal parenchyma or adjacent organ injuryin percutaneous renal access thulium laser treatmentgroup. The patients had nofever,no urinary tract infection,no leakage of urine and so on after operation. There wasno significant difference（ P〉 0.05） in the operation time, postoperative hospital stay, intraoperative andpostoperative complicationsbetween the percutaneous renal holmium laser group and the conventional laparoscopicgroup.Conclusion Percutaneous renal access thulium laser treatment of Bosniak stage I and II renal cysts isasafe, feasible and effective technique.
