摘  要:目的:应用电流感觉阈值(Current Perception Threshold,CPT)检测仪检测糖尿病患者周围小神经纤维,评估神经受损程度。方法:采用美国Neurotron公司CPT检测仪,选取50例有症状的糖尿病患者(糖尿病组)及健康者(正常组)的双侧正中、尺神经及腓深、腓浅神经进行对照分析,记录电极放在无名指及拇趾用三种不同频率(2 000Hz、250Hz、5Hz)的正弦波刺激进行测试,所测数据输入由其检测仪软件处理系统,自动分析出神经纤维受损程度。结果:双上肢正中神经、尺神经、双下肢腓浅、腓深神经的CPT检测,两组人员于2 000Hz、250Hz、5Hz不同频率刺激下所得CPT值进行比较,均显示P<0.01,两组之间有统计学差异。结论:CPT可补充常规运动感觉神经传导测定无法检测细有髓神经纤维及无髓神经纤维的缺陷,联合常规神经传导测定,可提高糖尿病神经病变的早期诊断率。Objective:The purpose of this research is to detect peripheral minor nerve fiber damage of diabetic patients and assess severity of damage by using Current Perception Threshold(CPT)detector.Methods:The CPT Detector from Neurotron company of USA was used to 50 patients and healthy peoplewho were selected respectively to compare and analyze their median nerve at both sides,ulnar nerve,deep peronealnerveand superficial peroneal nerve,the recording electrode of which was placed on the ring finger the hallux to test by using sine wave stimulation of 3 different frequencies(2 000 Hz,250 Hz and 5 Hz);then the captured data was entered for the software processing system of the detector to analyze and determine severity of nerve fiber damage automatically.Results:Through the CPT detecting to the double median nerve and ulnar nerve and the double lower limb superficial peroneal nerve and deep peroneal nerve,the value of CPT both patients and healthy people at 2 000 Hz,250 Hz and 5 Hz showed P〈0.01,in which there is significant difference.Conclusions:CPT can supply the defect which can not detect the peripheral minor nerve fibre,and it can help increase diagnosis rate of diabetic neuropathy as early as possible.
