摘 要:目的:研究应用自动化药品检测机对单剂量口服分包药品进行核对的成效。方法:应用药品检测机对25个连续工作日的单剂量口服分包药品进行实时检测,并对检测机所出的错误报告由药师进行二次核对,记录每次错误发生的原因和信息并加以分析。结果:25个连续工作日应用第二代药品检测机(Medicine Detection Machine,MDM)实时检测111 940包药包,共报告6 805份药品错误报告,错误上报率为6.08%。其中真错误发生率为0.22%,占到所有错误的3.62%;假错误发生率为5.86%,占到所有错误的96.38%。其中碎药的发生比例最高(约0.10%),占到了所有真错误的48.35%,其次是药包中多药渣,发生比例约为0.05%,占所有真错误的23.14%。假错误中,药片位置的原因导致的假错误占到了所有假错误的40.76%,其次是药品本身的原因和药包原因,分别占了31.72%和11.92%。结论:采用检测机对口服药品的单剂量药包进行检测能够切实提高分发给患者的单剂量药品的准确度,降低患者的用药风险,并且提高药师工作效率。OBJECTIVE To study the effectiveness of using medicine detection machine(MDM).METHODS Using the MDM to detect the oral unit-dose in25 consecutive working days.Pharmacist rechecked the reports produced by the machine to record and analyze the errors.RESULTS Totally 111 940 packets were detected by the MDM in 25 working days,and 6,505(6.08%)errors were reported.Among them,the true error rate was 0.22%and false error rate was 5.86%.Broken medicine(0.10%)and drug residue occurred(0.05%)were the two highest problems.Among the false errors,the location of the tablets was the most,followed by the reasons for the tablets themselves and the cause of the packets.CONCLUSION MDM can actually improve the accuracy of oral unit-dose dispensed to patients,reduce the patient’s medication risk,and improve efficiency of the pharmacist.
