摘 要:目的探讨某医院住院患者泌尿系统念珠菌感染的流行情况及耐药现状,为临床治疗提供参考。方法对2015年1月—2017年12月住院患者送检的尿液培养标本中分离的念珠菌进行菌种鉴定及药敏试验。结果 3663份尿液标本共培养出念珠菌175株,感染菌种以光滑念珠菌和白色念珠菌位居第一、二位,分别占41.7%、20.6%。药敏显示念珠菌对两性霉素B 100.0%敏感,对5-氟胞嘧啶、伏立康唑、氟康唑、伊曲康唑的耐药率分别为7.4%、34.3%、38.8%、59.4%。白色念珠菌的耐药率较低,非白色念珠菌耐药率比较高。结论该医院住院患者泌尿系统念珠菌感染以光滑念珠菌和白色念珠菌为主要病原菌,不同念珠菌对抗真菌药物的耐药性相差较大,临床应尽早进行病原学检测,根据药敏结果使用抗真菌药物。Objective To discuss the prevalence and drug resistance status of Candida infection in hospital patients' urinary system to provide reference for clinical treatment. Methods Do the bacteria identification and drug sensitivity test of Candida which separated from urine samples of patients in hospital in 2015 January to 2017 December. Results 175 strains of Candida were cultured from 3663 urine specimens, they are Candida glabrata and Candida albicans ranked the first, second, 41.7%, 20.6% respectively. Drug sensitivity showed Candida sensitive to Amphotericin B 100.0% and resistance rates to 5-Flucytosine, voriconazole, fluconazole, itraconazole were 7.4%, 34.3%, 38.8%, 59.4% respectively. The resistance rate of Candida albicans was lower than that of non Candida albicans. Conclusion Candida glabrata and Candida albicans are the main pathogens of Candida in inpatients in this hospital. The drugs resistance of Candida albicans is different from that of other fungi, so it is necessary to detect the pathogens in clinic as soon as possible and use antifungal drugs according to the results of drug sensitivity.
