细银质针导热联合羧甲基壳多糖关节内注射治疗膝骨性关节炎

摘　　要：目的:观察细银质针导热联合羧甲基壳多糖(几丁糖)关节内注射治疗膝骨性关节炎(knee osteoarthritis, KOA)的临床效果。方法:膝骨性关节炎病人90例,随机均分为细银质针导热联合几丁糖关节内注射组(S组,n=45)和几丁糖关节内注射组(C组,n=45)。S组采用细银质针导热联合几丁糖关节内注射,C组单纯几丁糖关节内注射。评价治疗前、治疗后4周、8周的Lequesne指数和WOMAC评分,记录治疗中和治疗后的并发症。结果:两组病人治疗前Lequesne指数和WOMAC评分无显著差异,治疗后4周和8周时的Lequesne指数和WOMAC评分明显低于治疗前基础值(P <0.05),但S组更为显著(P <0.05)。结论:细银质针导热联合几丁糖关节内注射能有效改善膝骨性关节炎的疼痛和功能障碍,是治疗膝骨性关节炎的有效方法。Objective： To observe the clinical effect of fine silver needle thermal conduction combined with intra-articular injection of chitosan in the treatment of knee osteoarthritis（KOA）. Methods： Ninety patients with knee osteoarthritis were randomly divided into fine silver needle thermal conduction combined with chitosan intra-articular injection group（S group, n = 45） and chitosan intra-articular injection group（C group, n = 45）. Patients in Group S were treated with fine silver needle thermal conduction combined with intra-articular injection of chitosan, and patients in group C were injected intra-articularly with chitosan. Lequesne index and WOMAC scores were evaluated before treatment, 4 weeks and 8 weeks after treatment, and complications during and after treatment, if any, were recorded. Results： There was no significant difference in Lequesne index and WOMAC scores between the two groups before treatment. The Lequesne index and WOMAC scores at 4 and 8 weeks after treatment were lower significantly than the pre-treatment baseline values（P〈 0.05）, but group S was more significant（P 〈0.05）. Conclusion： Fine silver needle combined with chitosan intra-articular injection can effectively relieve the pain and improve dysfunction of knee osteoarthritis, and it is an effective method for the treatment of knee osteoarthritis.
