摘　　要：目的探讨膝关节镜引导小切口术对胫骨平台骨折患者术后膝关节功能的影响。方法选择我院2015年2月至2016年3月收治的72例胫骨平台骨折患者为研究对象,以随机数字表法分为对照组与观察组,各36例。对照组实施传统内固定术治疗,观察组实施膝关节镜引导小切口内固定术治疗。比较两组患者术后恢复情况、并发症发生情况、膝关节功能及膝关节活动度。结果观察组术后引流量少于对照组,术后首次下床活动时间、住院时间、骨折愈合时间均明显短于对照组(P<0.05)。观察组术后并发症总发生率明显低于对照组(P<0.05)。随着术后时间的延长,两组膝关节Lysholm评分均逐渐升高,且观察组术后6、12个月评分均高于对照组(P<0.05)。观察组术后3、6、12个月的伸膝最大角度、屈膝最大角度均明显大于对照组(P<0.05)。结论膝关节镜引导小切口内固定术治疗胫骨平台骨折效果显著,可减少并发症的发生,改善患者膝关节功能,促进患者术后早日恢复,值得推广。Objective To explore the effect of knee arthroscopy guided small incision on knee joint function in patientswith tibial plateau fracture after operation. Methods Seventy-two patients with tibial plateau fracture admitted in our hospitalfrom February 2015 to March 2016 were selected as the research objects and divided into control group and observationgroup by random number table method, with 36 cases in each group. The control group was treated with traditional internalfixation, while the observation group was treated with knee arthroscopy guided small incision internal fixation. Thepostoperative recovery, complications, knee function and range of motion were compared between the two groups. ResultsThe postoperative drainage volume in the observation group was less than that in the control group, and the time ofpostoperative first ambulation, hospitalization and fracture healing were significantly shorter than those in the control group（P〈0.05）. The total incidence of postoperative complications in the observation group was significantly lower than that in thecontrol group（P〈0.05）. The Lysholm scores of knee joints in both groups increased gradually with the prolongation ofpostoperative time, and those in observation group were higher than the control group at 6, 12 months after operation（P〈0.05）.The maximum knee extension angle and flexion angle of the observation group were significantly higher than those of thecontrol group at 3,6 and 12 months after operation （P〈0.05）. Conclusion Knee arthroscopy guided small incision internalfixation in the treatment of tibial plateau fracture has a remarkable effect, which can reduce complications, improve kneefunction of patients, and promote early recovery of patients. It is worth promoting.
