植入式静脉输液港两种不同囊袋建立方式的对比研究

摘　　要：目的探讨采取锁骨下低位切口反向建立囊袋在静脉港植入术中的安全性和可行性。方法回顾性分析80例需行辅助化疗的乳腺癌患者的临床资料,均采用经颈内静脉穿刺,其中常规方式建立囊袋40例,锁骨下低位切口反向建立囊袋40例。分析比较两组术后并发症等方面有无差异。结果反向囊袋建立组和常规囊袋建立组在植港手术时间、术后疼痛、出血方面差异无统计学意义,反向囊袋建立组在外观满意度、平均取港时间方面优于常规囊袋建立组。结论锁骨下低位切口相对于传统手术切口较隐蔽,改善患者生活质量,方便静脉港取出,可选择性替代传统手术切口。Objective To explore the safety and feasibility of using the subclavian low incision to build the subcutaneous capsular bag in totally implantable venous access devices. Methods The clinical data of 80 patients with breast cancer who needed adjuvant chemotherapy were retrospectively analyzed. Puncture site of all patients through the internal jugular vein. 40 cases of subcutaneous capsular bag were established by traditional surgical approach. 40 cases were established by subclavian lower incision. Two groups of postoperative complications and other aspects of the difference were analyzed. Results There was no significant difference in the operation time,postoperative pain and bleeding between the observation group and control group. In the observation group,the appearance satisfaction and the average time of removing the port were excellent. Conclusion The lower subclavian incision is more concealed than traditional surgical incision,improving the quality of life of patients,to facilitating the removal of the port,and optionally replacing the traditional surgical incision.
