医联体内急诊学科松散型管理模式运行效果分析

摘　　要：目的探讨医联体内急诊学科松散型管理模式下,二级医院急诊学科管理措施与效果。方法实施医联体内急诊学科松散型管理模式,比较实施前四年和实施后四年二级医院急诊科专科能力、急诊质量指标情况。结果实施医联体内急诊学科松散型管理模式后,二级医院急诊专科能力明显提高,差异有统计学意义,P <0. 05;急诊质量指标除急救设备完好率、急救药品完好率外,其它5项质量指标明显提高,差异有统计学意义,P <0. 01或P <0. 05。结论在医联体内急诊学科松散型管理模式下,三级医院急诊科对二级医院急诊科进行专科帮扶指导,二级医院急诊科借鉴三级医院急诊科先进管理理念和方法,可以明显提升本学科专科能力和质量管理水平。Ojbective To explore operational effects of emergency medicine loose medical management mode in the integrated health care system （EMLMM）. Methods The EMLMM was put into practice for 4 years. The effects were compared between pre- and post-practice in respect of the hospitals’ ability and quality of emergency service. Results After the implementation of the mode in the integrated health care system, the service ability of emergency medicine in the affiliated hospitals significantly improved （ P 〈0.05）. Five quality-related indicators of emergency medicine service except the intactness rate of emergency equipment and first-aid medicine were also significantly enhanced （ P 〈0.01 or P 〈0.05）.Conclusion Under the implementation of EMLMM in the integrated health care system, the emergency department of the tertiary hospitals can offer the professional assistance and guidance to the secondary hospital. The emergency departments of the secondary hospitals can learn advanced management ideas and methods from the tertiary hospital, which can evidently boost the professional competence and quality management of the emergency department.
