摘 要:目的:分析35岁以下妇女获卵数对体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法:将行IVF-ET治疗的35岁以下女性病人依照获卵数分为3组,A组:获卵数≤10个,B组:获卵数11~20个,C组:获卵数> 20个,分析获卵数对IVF-ET妊娠结局的影响。结果:A组病人促性腺激素(Gn)使用时间及支数明显高于B组和C组(P <0. 01); C组Gn使用时间与B组差异无统计学意义(P> 0. 05),但Gn使用支数明显低于B组(P <0. 01); C组人绒毛膜促性腺激素治疗当日雌二醇水平明显高于B组(P <0. 01),B组明显高于A组(P <0. 01),各组受精率、优质胚胎率、临床妊娠率、种植率差异无统计学意义(P>0. 05)。卵巢过度刺激率随着获卵数增加而增加,但差异无统计学意义(P> 0. 05)。结论:临床提高IVF-ET胚胎着床率及临床妊娠率,需获取足够高质量卵母细胞,且获卵数在11~20个效果最佳,应针对不同人群制定个性化治疗方案。Objective: To analyze the effects of the number of oocytes on the pregnancy outcome of in vitro fertilization and embryo transfer( IVF-ET) in women under 35 years old. Methods: From June 2015 to January 2017,the women under 35 years old treated with IVF-ET were divided into the group A( the number of oocytes less than 10),group B( the number of oocytes human chorionic gonadotropin 11 to 20) and group C( the number of oocytes more than 20) according to the number of oocytes. The effect of the number of oocytes on the pregnancy outcome of IVF-ET was analyzed. Results: The use time and number of Gn in group A were significantly higher than those in group B and group C( P〈0. 01),the difference of the use time of Gn between group C and group B was not statistically significant( P〉0. 05),and the use number of Gn in group C was significantly lower than that in group B( P〈0. 01). The estradiol level in group C treated with human chorionic gonadotropin was significantly higher than that in group B( P〈0. 01),and which in group B was significantly higher than that in group A( P〈0. 01). The differences of the fertilization rate,high quality embryo rate,clinical pregnancy rate and planting rate among three groups were not statistically significant( P〉0. 05). With the increasing of the number of oocytes,the rate of ovarian hyperstimulation increased,and the difference of which was not statistically significant( P〉0. 05). Conclusions: In order to improve the implantation rate and clinical pregnancy rate of IVF-ET,the high-quality oocytes should enough be obtained,the effect of 11 to 20 oocytes is the best,and the individual treatment plan for different pregnant women should be formulated.
