摘  要:目的:探讨踝臂指数(ankle brachial index,ABI)、颈动脉内膜中层厚度(intima-media thickness,IMT)与早发冠心病(intima-media thickness,PCAD)病人冠脉病变严重程度的相关性及对早发冠心病的早期预测价值。方法:选择136例拟诊p CAD收住行冠脉造影(coronary angiography,CAG)的病人,均行ABI、颈动脉IMT检查,根据冠脉狭窄程度计算冠脉积分,分析ABI、颈动脉IMT值与冠脉积分的相关性。结果:单支与双支病变组病人ABI、颈动脉IMT差异均无统计学意义(P> 0. 05),双支病变组Gensini积分高于单支病变组(P <0. 05);三支病变组病人ABI均明显低于其他3组,颈动脉IMT均明显高于其他3组,Gensini积分均明显高于单支和双支病变组。ABI值与冠脉Gensini积分呈负相关关系(r=-0. 523,P <0. 01);颈动脉IMT值与冠脉Gensini积分成正相关(r=0. 603,P <0. 01)。结论:ABI、颈动脉IMT与p CAD病人冠脉病变严重程度有一定相关性,ABI值明显减低、颈动脉IMT明显增加可能预测冠脉存在严重多支病变。Objective: To investigate the correlations of ankle brachial index( ABI) and intima-media thickness( IMT) with severity of coronary lesion in premature coronary artery disease( p CAD),and its early predictive value on p CAD. Methods: The ABI and carotid artery IMT in 136 p CAD patients detected by coronary angiography( CAG) were measured. The coronary score was calculated according to the degree of coronary stenosis,and the correlations of ABI and IMT with coronary score were analyzed. Results: The differences of the ABI and carotid artery IMT between single-and double-vessle lesion patients were not statistically significant( P〉0. 05),and the Gensini score in double-vessle lesion group was higher than that in single-vessle lesion group( P〈0. 05). The ABI and carotid artery IMT in three-vessle lesion group were significantly lower and higher than which in other groups,respectively( P〈0. 01),and the Gensini score in three-vessle lesion group was significantly higher than that in single and double-vessle lesion groups( P〈0. 05). The ABI value was negative correlation with the Gensini score of coronary artery( r =-0. 523,P〈0. 01),and the carotid artery IMT was positive with correlation with the Gensini score of coronary artery( r = 0. 603,P〈0. 01). Conclusions: The ABI and carotid artery IMT have certain correlation with severity of coronary lesions in p CAD patients. The ABI significant decreasing and carotid artery IMT significant increasing can predict the severity of coronary lesion in p CAD patients.
