摘 要:目的:探讨山东省女性尖锐湿疣患者人乳头状瘤病毒(human papillomavirus,HPV)感染和不同亚型分布情况。方法:选择2012年1月至2017年5月在济南金域医学检验中心经病理诊断为尖锐湿疣的795例女性患者为研究对象,回顾性分析其HPV23基因分型检测结果。患者主要来自于山东省不同地区约200余家医院的性病和皮肤科门诊。结果:730例女性HPV检测阳性,阳性率为91. 8%。低危亚型前3位依次为HPV6(46. 0%)、11(45. 5%)和43(5. 1%);高危亚型前5位依次为HPV16(15. 5%)、58(8. 9%)、52(8. 8%)、51 (7. 1%)和59 (6. 4%)。单重感染267例(36. 6%),二重及以上感染(多重感染) 463例(63. 4%)。在单重感染中,HPV31、35、52、59、73和82未有检出,而多重感染中23种HPV亚型均有检出。不同年龄分布(<30岁、30岁~49岁和≥50岁)显示,在30岁~49岁组中,单重感染(47. 3%)和单纯低危感染(44. 7%)占比最高,而高低危混合感染(42. 5%)占比则最低;四重及以上感染分别在≥50岁(29. 7%)和<30岁组(18. 7%)占比最高。此外,单纯低危感染随着感染重数增加占比明显降低,而高低危混合感染在二重感染中占比最低,后随感染重数增加占比明显增加。结论:本研究结果反映了山东省女性尖锐湿疣患者HPV不同亚型感染的分布情况,为山东省女性HPV感染的治疗和预防提供了科学依据。Objective: To investigate the prevalence and genotype distribution of human papillomavirus( HPV) infection among women with condyloma acuminatum( CA) in Shandong Province. Methods: From January2012 to May 2017,795 female patients with CA referred to Jinan King Med Diagnostics were diagnosed by histopathology and tested by a HPV genotyping panel,which could detect 23 HPV genotypes. The prevalence and genotype distribution of HPV infection were documented and analyzed. Most of the patients were from about 200 outpatient services of venereal disease or department of dermatology in different regions of Shandong Province. Results: A total of 730 cases were HPV-positive and the positive rate was 91. 8%. HPV6( 46. 0%),11( 45. 5%) and 81( 5. 1%) were the most three common low-risk HPV( lr HPV) genotypes. HPV16( 15. 5%),58( 8. 9%),52( 8. 8%),51( 7. 1%) and 59( 6. 4%) were the most five common high-risk HPV( hr HPV) genotypes. Of these HPV-positive patients,267 cases( 36. 6%) had single genotype infections,and the other 436 cases( 63. 4%) had double or more genotype infections( multiple genotype infections). Beyond HPV31,35,52,59,73 and 82,the other HPV subtypes were detected in single subtype infections. All 23 HPV subtypes were detected in multiple infections. Age-specific( 〈30 years,30-49 years and ≥50 years) distributions showed that single genotype infections( 47. 3%) and pure lr HPV infections( 44. 7%) had the highest proportions,while mixed lr HPV and hr HPV infections( 42. 5%) had the lowest proportions in women of 30-49 years old. Moreover,the highest proportions of≥4 genotype infections were detected in women ≥50 years( 29. 7%) and 〈30 years( 18. 7%),respectively. As the number of infections increased,the proportion of pure lr HPV infections was decreased,while the proportion ofmixed lr HPV and hr HPV infections was increased.
