声导抗咽鼓管功能检测在分泌性中耳炎诊疗中的应用

摘　　要：目的探讨声导抗咽鼓管功能检测在分泌性中耳炎(secreturyotitismedia,SOM)诊疗中的应用价值。方法选取我科2017年3月～2018年3月SOM患者60例,其中儿童(<14岁)患者35例/58耳(单耳12例,双耳23例),成年患者25例/35耳(单耳15例,双耳10例)。分别在首次就诊、治疗后1周及治疗后1月对受试者进行声导抗咽鼓管功能测试等听力检测,对其听力曲线、鼓室图及咽鼓管功能进行分析。结果治疗前,中耳功能异常耳为82.50%,咽鼓管功能异常耳(欠佳+不良)为84.17%。治疗后,中耳功能异常耳17.50%,咽鼓管功能异常耳为35.00%。结论咽鼓管功能与中耳鼓室导抗图两者间具有相关性;咽鼓管功能得以改善者,SOM恢复较好。对于病程<3个月的患者,咽鼓管功能改善较病程>3月患者佳。Objective To explore the application value of acoustic impedance in the diagnosis and treatment of secretory otitis media （SOM）. Methods A total of 60 SOM patients were selected from March 2017 to March 2018. Among them,35 patients/58 ears （12 single-ear, 23 binaural） were children （〈14 years old） and 25 adult patients/35 ears （15 single- ear,10 binaural）. At the time of their first visit, one week after treatment and one month after treatment, the subjects received hearing tests,including acoustic Eustachian tube functional tests, and the results of their hearing thresholds tympanograms, and Eustachian tube functions were analyzed. Results Before treatment, there were 82.50% of ears with abnormal middle ear function and 84.17% of ears with abnormal Eustachian tube function （subpar/ unhealthy）. After treatment, there were 17.50% of ears with abnormal middle ear function and 35.00% of ears with abnormal Eustachian tube function. Conclusions Eustachian tube function is related to the tympanogram of the middle ear tympanometry; the SOM function is improved and SOM recovery is better. For patients with a disease course 〈3 months, the improvement of Eustachian tube function was better than that of patients with a disease course 〉3 months.
