我院门诊82例退药处方原因分析与干预措施

摘　　要：目的分析我院门诊82例退药处方原因与干预措施。方法选择2016年1月-2018年2月我院门诊退药事件82例,对所有的退药申请单进行回顾性分析,根据退药的品种类型、相关科室及退药的原因等进行分类、整理和记录。结果 82例退药事件原因如下:(1)药品因素40例(48. 78%):药物不良反应30例、药房供应短缺4例、药品接近有效期4例、收费错误2例;(2)医师因素22例(26. 83%):医师处方错误5例、同类药品重复开药5例、未注意药物配伍禁忌4例、患者收治入院4例、沟通不足4例;(3)患者因素20例(24. 39%):自认为不良反应多不愿使用10例、服药依从性差6例、费用问题2例、家中有备药2例。结论导致门诊药房退药现象的原因和影响因素较复杂,要求临床医师和药师针对退药的影响因素进行针对性分析,从而最大限度降低门诊药房的退药率,实现药物治疗的合理、经济、有效、安全。Objective To discuss the causes and intervention measures of 84 prescription withdrawal prescriptions in our hospital. Methods 82 cases of drug withdrawal were selected from outpatients in our hospital from January 2016 to February 2018, and all drug withdrawal applications were retrospectively analyzed. According to the type of drug withdrawal, the relevant departments and the reasons for drug withdrawal, classification and finishing were carried out and recorded. Results The causes of 82 cases of drug withdrawal were as follows ： （ 1 ）40 cases of drug factors（48.78% ） ：30 cases of adverse drug reactions ,4 cases of shortage of pharmacy supply ,4 cases of drug near valid period ,2 cases of wrong charges ; （2） 22 eases of physician factors （26.83%） ：5 cases of doctor＇s prescription error, 5 cases of repeated drug treatment of similar drugs,4 cases of drug incompatibility ,4 cases of patients admitted to hospital ,4 cases of communication; （3）20 cases of patients （24.39%） ： Unwilling to use 10 cases of adverse reactions,6 cases of poor medication compliance ,2 cases of cost problems,and 2 cases of home preparation. Conclusion The causes and influencing factors of drug withdrawal in outpatient pharmacies are complicated. It is required that clinicians and pharmacists conduct targeted analysis on the influencing factors of drug withdrawal, so as to minimize the drug withdrawal rate nf nutnatient pharmacies and realize rational.economic effective and safetv drug treatment.
