腹腔镜下二次胆管手术治疗胆总管结石的临床效果及安全性

摘　　要：目的探讨腹腔镜下二次胆管手术治疗胆总管结石的临床效果及安全性。方法选取景德镇市第二人民医院2015年8月—2017年11月收治的需二次手术的胆总管结石患者40例,随机分为对照组和试验组,各20例。对照组患者采用传统手术方式治疗,试验组患者采用腹腔镜手术治疗。比较两组患者手术时间、术中出血量、住院时间、胃肠功能恢复时间及不良反应发生情况,并比较两组患者结石取出率。结果试验组患者手术时间、住院时间及胃肠功能恢复时间短于对照组,术中出血量少于对照组(P<0.05)。试验组患者不良反应发生率低于对照组(P<0.05)。试验组患者结石取出率高于对照组(P<0.05)。结论腹腔镜下二次胆管手术治疗胆总管结石的临床效果确切,其可有效缩短患者治疗时间,减少术中出血量,提高结石取出率,且安全性高。Objective To study the clinical effect and safety of laparoscopic secondary biliary tract surgery in the treatment of choledocholithiasis.Methods A total of 40 cases of choledocholithiasi patients with secondary biliary tract surgery were selected from August 2015 to November 2017 in Jingdezhen Second People＇s Hospital,which were randomly divided into control group and experimental group,20 cases in each group.The control group was given traditional surgical treatment,experimental group was given laparoscopic choledocholithotomy.The operation time,bleeding volume,hospitalization time,recovery time of normal function and adverse reaction were compared between the two groups,and the stone removed rate was compared between the two groups.Results The experimental group of operation time,hospitalization time,recovery time of normal function were shorten than control group,bleeding volume was less than control group（P0.05）.The incidence rate of adverse reaction in experimental group was lower than control group（P0.05）.The experimental group of stone removed rate was lower than control group（P0.05）.Conclusion Aparoscopic secondary biliary tract surgery has an exact clinical effect in the treatment of choledocholithiasis,it can effectively shorten treatment time of patients,reduce bleeding volume,and promote stone removed rate,which is safe.
