摘  要:目的观察皮肤再生医疗技术(moist exposed burn therapy/moist exposed burn ointment,MEBT/MEBO)对Wagner2级糖尿病足患者密歇根神经病变筛选表(MNSI)评分及患肢神经肌肉电生理的影响。方法将2013年10月—2016年6月于右江民族医学院附属医院被初次诊断和治疗的52例2型糖尿病合并Wagner2级糖尿病足患者按照入院先后顺序采用单双号数字表法随机分为MEBO组(26例)与贝复新组(26例),其中MEBO组患者采用湿润烧伤膏(moist exposed burn ointment,MEBO)治疗局部创面,贝复新组患者采用重组牛碱性成纤维细胞生长因子凝胶(贝复新)治疗局部创面。对比观察两组患者入院时及治疗14、28 d时的MNSI评分、腓总神经运动传导速度(MCV)及感觉传导速度(SCV)、腓肠肌运动单位电位(MUP)时限。结果治疗过程中,MEBO组与贝复新组患者MNSI评分及腓肠肌MUP时限均逐渐降低、腓总神经MCV与SCV均逐渐升高,其中各时间点MNSI评分、腓肠肌MUP时限及腓总神经MCV与SCV组内两两对比,P均<0. 05,差异具有统计学意义;治疗14、28 d时MEBO组与贝复新组患者MNSI评分、腓总神经MCV与SCV、腓肠肌MUP时限对比,P均> 0. 05,差异无统计学意义。结论 MEBT/MEBO可促进Wagner2级糖尿病足创面的神经修复,改善周围神经传导速度,疗效与具有神经再生作用的贝复新相近,值得临床推广应用。Objective To observe the effects of Moist Exposed Burn Therapy/Moist Exposed Burn Ointment( MEBT/MEBO) on the Michigan Neuropathy Screening Instrument( MNSI) score and neuromuscular electrophysiology of patients with Wagner grade 2 diabetic foot. Methods Fifty-two patients with type 2 diabetes mellitus combined with Wagner grade 2 diabetic foot,initially diagnosed and treated at the Affiliated Hospital of Youjiang Medical University for Nationalities from October 2013 to June 2016,were randomly divided into MEBO group( 26 cases) and bF GF group( 26 cases)according to the odd-even number table of their admission sequence. Wounds of patients in the MEBO group were treated with Moist Exposed Burn Ointment( MEBO),while wounds of patients in the bF GF group were treated with recombinant bovine basic fibroblast growth factor gel( bF GF). Michigan Neuropathy Screening Instrument( MNSI) score,Motor conduction velocity( MCV) and sensory conduction velocity( SCV) of common peroneal nerve,and the gastrocnemius motor unit potential( MUP) duration were compared between the two groups at the time of admission and on day 14 and 28 of treatment. Results During the treatment course,both in the MEBO group and the bF GF group,the MNSI score and the gastrocnemius MUP duration gradually decreased while the MCV and SCV of common peroneal nerve gradually increased. The pairwise comparisons of MNSI score,gastrocnemius MUP duration,and MCV and SCV of common peroneal nerve at different time points within each of the two groups all showed statistically significant differences( all P〈0. 05). MNSI score,MCV and SCV of common peroneal nerve,and gastrocnemius MUP duration were compared between the two groups respectively on day 14 and 28 of treatment and the results showed no statistically significant difference( all P〈0. 05). Conclusion MEBT/MEBO can promote the repair of nerves in Wagner grade 2 diabetic foot wounds and improve the peripheral nerve conduction velocity. Its curative effect i
