微创钢板接骨技术联合锁定加压钢板内固定治疗四肢粉碎性骨折的临床效果

摘　　要：目的 探讨微创钢板接骨技术（MIPPO）联合锁定加压钢板内固定（LCP）对四肢粉碎性骨折患者术后骨折的疗效.方法 选择2017年1月至2017年12月临汾市人民医院四肢粉碎性骨折患者90例做为观察对象,在患者自愿和医生建议的前提下将患者分为常规切开复位内固定术治疗组（对照组）和MIPPO联合LCP治疗组（观察组）,每组45例.比较两组患者的手术情况、术后并发症发生率及临床疗效等指标.结果 观察组的手术时间、失血量、住院时间及骨折愈合时间低于对照组,差异均有统计学意义（t=4.375,P=0.003;t=4.895,P=0.002;t=3.982,P=0.003;t=4.871,P=0.001）.观察组术后并发症发生率明显低于对照组,差异有统计学意义（χ2=4.577,P=0.002）.观察组的疗效优于对照组,差异有统计学意义（χ2=4.894,P=0.002）.结论 MIPPO联合LCP治疗四肢粉碎性骨折具有较好的临床疗效,手术切口创伤小,手术时间短、术后并发症少、恢复速度快,给患者带来的疼痛较轻,更有利于骨折的愈合,具有快速、有效、愈合率高及稳固性好的临床效果,临床应用价值较高.Objective To investigate the curative effect of the minimally invasive plate osteosynthesis （ MIPPO） and locking compression plates （ LCP ） on comminuted fracture of four limbs . Methods A total of 90 patients with comminuted fracture of four limbs in Linfen People ＇s Hospital from January 2017 to December 2017 were recruited as the study objects .They were randomly divided into conventional open reduction and internal fixation in treatment group （ control group ） and MIPPO in conjunction with LCP group （ observation group ） on the premise of the voluntary of patients and the advice of doctors , with 45 patients in each group . The operation condition , incidence of postoperative complications and clinical efficacy in the two groups were compared .Results The operation time , blood loss, hospital stay, and fracture healing time in observation group were significantly lower than those in control group, and the differences were significant （ t=4.375, P=0.003; t=4.895, P=0.002; t=3.982, P=0.003;t=4.871, P=0.001）.The incidence of postoperative complications in observation group was significantly lower than that in control group , and the difference was significant （χ2 =4.577, P=0.002 ） .The effect in the observation group was significantly better than that in control group , and the difference was significant （χ2 =4.894, P=0.002）.Conclusions Internal fixation of MIPPO and LCP have better clinical efficacy on comminuted fracture of four limbs , the surgical incision is small , the operation time is short , the postoperative complications is little , the recovery is fast , the pain is less , which is more conducive to the fracture healing .It has the clinical fast , efficient, high healing rate and good stability , and is of high clinical value in clinical application .
