老年急性左心功能不全合并呼吸衰竭患者早期应用无创呼吸机通气的临床疗效

摘　　要：目的 探讨老年急性左心功能不全（简称急性左心衰）合并呼吸衰竭患者早期应用无创呼吸机通气的效果.方法 选取2017年3月至2018年5月山西省运城市中心医院就诊的老年急性左心衰合并呼吸衰竭患者86例,根据随机数表法分为对照组和观察组,每组43例.对照组采用持续低流量吸氧,观察组给予早期无创呼吸机通气治疗,比较两组血气分析指标、心功能指标及不良事件发生情况.结果 与对照组比较,观察组治疗后动脉血氧分压（PaO2）、血氧饱和度（SaO2）水平较高,动脉血二氧化碳分压（PaCO2）水平较低,差异有统计学意义（P〈0.05）.观察组治疗后左心室射血分数（LVEF）、每搏输出量（SV）水平均高于对照组,差异有统计学意义（P〈0.05）.两组治疗期间不良事件发生率比较,差异未见统计学意义（P〉0.05）.结论 早期无创呼吸机通气治疗可有效改善老年急性左心衰合并呼吸衰竭患者的血气指标及心功能.Objective To investigate the effect of early application of noninvasive ventilator ventilation on elderly patients with acute left heart failure and respiratory failure .Methods Eighty-six patients with acute left heart failure combined with respiratory failure from March 2017 to May 2018 in the Central Hospital of Yuncheng were selected .The patients were divided into control group and observation group according to the random number table method , with 43 cases in each group .The control group was treated with continuous low flow of oxygen inhalation , and observation group was given early non-invasive ventilation therapy .The blood gas analysis index , cardiac function index and adverse events in the two groups were compared .Results Compared with control group , in observation group , the levels of arterial oxygen partial pressure （ PaO2 ） and oxygen saturation （ SaO2 ） were higher, and the level of partial pressure of carbon dioxide was lower , and differences were significant （ P〈0.05 ） .The left ventricular ejection fraction （LVEF） and the output of stroke volume （SV） in observation group were higher than those in control group （ P〈0.05 ） , and there was no significant difference in the incidence of adverse events between the two groups （ P〉0.05 ） .Conclusions Early non-invasive ventilator ventilation can effectively improve the blood gas index and cardiac function in elderly patients with acute left heart failure and respiratory failure .
