摘 要:目的:探讨16排螺旋CT低管电压联合迭代重建技术应用于头颈部血管成像的可行性。方法 :收集连续16例行头颈部CTA检查的患者,采用Philips Ingenuity Flex 16排CT(90 kV)行头颈部扫描,分别记录CT剂量报告中的剂量长度乘积(DLP)。将原始图像分别采用滤波反投影法(FBP)、迭代重建技术iDose 1、iDose 2、iDose 3、iDose 4、iDose 5重建,共获得6组图像并进行治疗评价。客观图像质量评价包括测量各组主动脉、颈总动脉、颈内动脉和大脑中动脉的CT值、噪声、SNR和对比信噪比,并进行方差分析或非参数Kruskal-Wallis H检验。图像质量主观评价由2名有经验的影像诊断医师对各组图像采用5分法评价图像锐利度、噪声。结果:各组CT值差异无统计学意义(P> 0.05)。迭代重建技术iDose 1、iDose 2、iDose 3、iDose 4、iDose 5图像噪声、SNR和对比信噪比普遍优于FBP重建图像。在主动脉节段CTA图像中,上述指标差异有统计学意义(P<0.05)。在迭代重建图像中,以iDose 5图像质量最佳。主观质量评价结果显示无3分以下的重建图像,所有图像质量均满足临床要求。结论:在16层螺旋CT(90 kV)头颈部CTA成像中,运用迭代重建技术能有效提高图像质量。Objective:To study the feasibility of 16-slice CT angiography in cerebral and carotid arteries with low tube voltage and iterative reconstruction technique. Methods:Sixteen consecutive patients underwent cerebral and carotid CT angiography by 16-slice CT with tube voltage 90 kVp,Iohexol (300 mgI/mL) as contrast medium. CT dose index (DLP) was recorded. CT im- ages were respectively reconstructed with filtered back projection (FBP) and iterative reconstruction (iDose 1,iDose 2,iDose 3, iDose 4 and iDose 5). The objective evaluation of CTA image quality included the measurement of CT attenuation of vessel lumen (including aorta,common carotid artery,internal carotid artery and middle cerebral artery),noise,signal-noise ratio and contrast signal-noise ratio, variance analysis or non-parameter Kruskal-Wallis H test were used to test the difference in different images. The subjective evaluation of CTA image quality was obtained by 2 experienced radiologists using double blind method with five scale score. Results:There was no significant statistic difference in CT values of blood vessel lumen in images recon- structed by different method. Noise,signal-noise ratio and contrast signal-noise ratio were improved in image reconstructed by it- erative reconstruction. In all images reconstructed by iterative reconstruction,images reconstructed by iDose 5 had the highest quality. In subjectively evaluated image quality,no image scale score was less than 3 point, and all images met the demand of clinical diagnosis. Conclusion:in cerebral and carotid CTA by 16-slice CT with low tube voltage,iterative reconstruction tech- nique can improve image quality.
