摘　　要：目的:描述抗菌药物潜在的药物相互作用(p DAAIs)在门诊患者中的发生频率和类型,并提出建议,促进临床用药有效安全。方法:以Medscape药物相互作用数据库为基础鉴别p DAAIs,并对p DAAIs的严重性和临床意义进行评估和分析,提出具体的药学干预建议。结果:本研究共纳入1 415例患者,审核了3 619条药物医嘱,鉴别和分析了165对p DAAIs。p DAAIs的发生与患者使用药物的数量相关。最常见的p DAAIs为克拉霉素联合甲硝唑,口服头孢菌素类药物联合抗酸药和喹诺酮类药物联合茶碱类药物。最常见的p DAAIs为的药学干预为监测药物临床疗效或毒性,避免联合使用和使用替代药物。结论:数据显示门诊患者中p DAAIs发生率较高,门诊患者使用抗菌药物需要药师通过药学干预来提高抗菌药物使用的安全性和有效性。Objective： To investigate the frequency and type of potential drug-antimicrobial agent interactions（p DAAIs） among outpatients,and to make recommendations to improve the medication safety and effectiveness of patients.Methods：Based on the Medscape database drug interactions,p DAAIs were identified and classified into categories of potential clinical outcome and management advice. Results：A total of 1 415 patients were enrolled in this study. There were3 619 medication and 420 potential DDIs were identified and analyzed. The occurrence of potential drug interactions was associated with the number of medications administered. The most common identified potential interacting pairs were metronidazole with clarithromycin,oral cephalosporins with antacids and quinolones with insulin or oral antidiabetics. Risk for adverse interactions could be managed by close monitoring clinical efficacy or toxicity of drugs,therapeutic alternative,and avoiding combinations. Conclusion：Data show that the incidence of p DAAIs among outpatients is high. Clinical pharmacists can intervene in p DAAIs,so as to improve medication efficacy and safety of outpatients.
