空气辅助膀胱镜检查在门诊活动性血尿患者中的诊断价值

摘　　要：目的:探讨空气辅助膀胱镜检查在门诊活动性血尿患者诊断中的价值。方法:选取我院门诊收治的168例活动性血尿患者,肉眼血尿患者行常规生理盐水灌注膀胱镜检查,如因出血影响视野导致膀胱镜检查失败,则改用空气辅助膀胱镜检查,观察膀胱各壁、前列腺及尿道等情况。结果:本组患者168例,其中119例使用生理盐水膀胱镜检查成功;49例失败后改用空气膀胱镜检查,其中47例检查成功。空气辅助膀胱镜检查成功率为95.9%,高于生理盐水灌注膀胱镜检查的70.8%(χ~2=51.06,P<0.05)。患者术中术后均无相关并发症。结论:空气辅助膀胱镜检查安全、可靠,可以提高血尿患者的膀胱镜一次检查成功率,值得在临床推广应用。Objective：To investigate the diagnosis value of air assisted cystoscopy and its clinical application in the diagnosis of adult outpatients with active gross hematuria.Methods：A total of 168 adult outpatients（106 male patients and 62 female patients）with active gross hematuria accepted routine cystoscopy using saline irrigation were selected in this study.If visualization during cystoscopy were impaired as the irrigation fluid became blood-stained,routine saline cystoscopy were converted to air assisted cystoscopy to get adequate visualization of the bladder wall,prostate and urethra.Results：There were 119 patients succeeded in the diagnosis of active hematuria with routine saline cystoscopy.49 patients failed with routine saline cystoscopy because of blood-stained irrigation saline and impaired visualization,and were converted to air adjunct cystoscopy.47 patients succeed with air adjunct cystoscopy.The success rate of one cystoscopy examination of sequential air assisted cystoscopy（95.9%）was higher than that of routine saline irrigation cystoscopy 70.8%（χ~2=51.06,P〈0.05）.There were no air cystoscopy related complications during operative and postoperative periods.Conclusion：Air assisted cystoscopy is a safe and effective technique that provides a good endoscopic view,and have considerable advantages in the clinical diagnosis of adult outpatients with hematuria.
