摘 要:目的探究妊娠期高血压疾病患者血清中同型半胱氨酸(Hcy)、胎盘生长因子(PLGF)、D-二聚体(D-D)的表达及相互关系,为临床诊治提供参考依据。方法选取98例妊娠期高血压疾病患者和25例健康孕妇;检测所有受试者收缩压(SBP)、舒张压(DBP)以及血清中总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)的水平,检测所有受试者血清中Hcy、PLGF、D-D的水平,并分析Hcy、PLGF、D-D与SBP、DBP、TC、TG、LDL-C、HDL-C的相关性。结果妊娠期高血压组、先兆子痫组、子痫组Hcy、D-D的水平显著高于正常对照组,差异有统计学意义(均P<0. 05),子痫组血清中Hcy、D-D的水平显著高于先兆子痫组和妊娠期高血压组,差异有统计学意义(均P<0. 05);且妊娠期高血压疾病患者血清中Hcy、D-D与SBP、DBP、TG、TC、LDL-C呈正相关,与HDL-C呈负相关。妊娠期高血压组、先兆子痫组、子痫组PLGF的水平显著低于正常对照组,差异有统计学意义(均P<0. 05),子痫组血清中PLGF的水平显著低于先兆子痫组和妊娠期高血压组,差异有统计学意义(均P<0. 05);且妊娠期高血压疾病患者血清中PLGF与SBP、DBP、TG、TC、LDL-C呈负相关,与HDL-C呈正相关。结论 Hcy、PLGF、D-D与妊娠期高血压疾病的发生、发展密切相关,与患者的血压和血脂指标存在相关性,因此可以作为妊娠期高血压疾病诊断和治疗的血清标志物。Objective To investigate the expression and correlation of serum homocysteine (Hcy), placental growth factor (PLGF) and D-dimer (D-D) in patients with hypertensive disorder complicating pregnancy, in order to provide reference for clinical diagnosis and treatment. Methods Selected 98 cases of patients with hypertensive disorder complicating pregnancy and 25 healthy pregnant women ; de- tected the systolic blood pressure (SBP) , diastolic blood pressure (DBP) and serum contents of total cholesterol (TC), triglyceride (TG) , low density lipoprotein cholesterol (LDL-C) , high density lipoprotein cholesterol (HDL-C) , and the levels of serum Hcy, PLGF, D-D in all the subjects, and analyzed the correlation between Hcy, PLGF, D-D and SBP, DBP, TC, TG, LDL-C, HDL-C. Results The levels of Hcy and D-D in gestational hypertension group, preeclampsia group and eclampsia group were significantly higher than those in normal control group, the differences were statistically significant ( P〈0. 05 ) ; the levels of serum Hcy and D-D in eclampsia group were sig- nificantly higher than those in preeclampsia group and hypertensive group, the differences were statistically significant ( P〈0. 05 ) ; the lev- els of serum Hcy and D-D in patients with pregnancy induced hypertension were positively correlated with SBP, DBP, TG, TC and LDL- C, and negatively correlated with HDL-C. The level of PLGF in gestational hypertension group, preeclampsia group and eclampsia group was significantly lower than that in normal control group, the difference was statistically significant ( P〈0. 05 ) , and the level of PLGF in ec- lampsia group was significantly lower than that in preeclampsia group and gestational hypertension group, the difference was statistically sig- nificant (P〈O. 05) ; the serum PLGF in patients with pregnancy induced hypertension was negatively correlated with SBP, DBP, TG, TC and LDL-C, and positively correlated with HDL-C.
