郁仁存治疗食管癌经验撷要

摘　　要：郁仁存教授认为食管癌的发生主要因痰、气、瘀交阻于食管,形成肿块,积久成癌。中医药治疗以清热解毒、祛瘀化痰散结、通道消噎为主。一方面,健脾养后天,补肾益先天,根据食管癌的肿瘤分期、治疗阶段以及患者的体质状况,决定以其为主或为辅;另一方面,使用活血化瘀、解毒散结中药消除肿瘤的同时,应用具有开通食管作用的药物,缓解症状。中医药以扶正为主,以达到减毒增效的目的,西医阶段性治疗结束后应以中医药长期巩固维持治疗,改善机体内环境,扶正与祛邪结合,防止复发转移。Professor YU Reneun believes that the occurrence of esophageal cancer is mainly caused by intermingle and blockage of phlegm,qi and stasis in the esophagus,forming a mass and gradually becoming cancer. Traditional Chinese medicine（TCM） treatment is mainly for clearing heat and removing toxin,removing stasis,resolving phlegm,dissipating mass,unblocking the esophagus and eliminating dysphagia. On the one hand,whether invigorating spleen to nourish the postnatal or tonifying kidney to benefit the prenatal should be the major treatment,which can be determined according to the tumor staging,treatment stage and pa- tients＇ physical condition of esophageal cancer;on the other hand,besides using Chinese herbal medicinals for activating blood and resolving stasis,removing toxin and dissipating mass to eliminate tumor,Chinese herbal medicinals for unblocking esophagus and relieving symptoms should also be applied. TCM is mainly based on reinforcing healthy qi to achieve the purpose of reducing toxicity and increasing efficiency. After the treatment of western medicine,TCM should be applied to consolidate and maintain treatment effect for a long time,to improve the internal environment of the body. Reinforcing the healthy qi and dispelling evil qi should be combined to prevent recurrence and metastasis.
